摘要
目的:观察复发性流产患者血清Th1/Th2细胞因子失衡及补肾固冲汤加减干预作用。方法:随机将复发性流产患者120例分为西药组和中药组,选取正常妊娠患者60例,做空白对照组。空白对照组不给予任何措施;西药组给予黄体酮注射液肌肉注射,15 mg/次,3次/周;中药组给予黄体酮注射的同时补肾固冲汤加减口服,100m L/次,日2次。分别于0、2、4、6、8、10、12周检测血清HCG、孕酮水平,观察各组患者流产情况。并于治疗前后检测各组患者血清TNF-α、IL-4细胞因子含量,观察Th1/Th2细胞因子免疫平衡情况。结果:治疗前,复发性流产患者血清HCG、孕酮水平明显低于空白对照组,经治疗后,水平均有不同程度上升,以中药组效果较为明显;计算各组有效率(分娩率)、中药组有效率(分娩率)明显升高;治疗前,与空白对照组比较,西药组和中药组Th1/Th2平衡明显向Th1型偏移;治疗后,西药组和中药组Th1/Th2平衡明显向Th2型偏移。结论:复发性流产患者血清Th1/Th2细胞因子明显向Th1型方向失衡,补肾固冲汤加减能有效提高临床治愈率,并使Th1/Th2细胞因子明显向Th2型偏移。
Objective：To observe the imbalance of serum Thl/Th2 cytokines in patients with recurrent spontaneous abortion and the effect of the intervention on the Bushen Guehong Decoction. Methods ： Randomly 120 cases of recurrent abortion were divided into Western medicine group and Chinese medicine group,60 cases of normal pregnancy were selected as blank control group. Blank control group were not giveany measures, the Western medicine group were given intramuscular injection of progesterone injection, 15 mg/time, 3 times/week, the Chinese medicine group was treated by injecting progesterone and Bushen Guchong Decoction, 100 mL/times,2 times a day. Serum levels of HCG and progesterone in serum at 0, 2,4,6,8,10 and 12 weeks, to observe the abortion of patients in each group. Before and after treatment, the serum levels of TNF- at and IL-4 were detected in all groups, observation of the immune balance of Th1/Th2 eytokines. Resuhs： Before treatment, the levels of serum HCG and progesterone in the patients with recurrent abortion were significantly lower than that in the control group, after treatment, the levels were increased to different extent, the effect of the traditional Chinese medicine group was more obvious. To calculate the efficiency of each group （ labor rate ）, the efficiency of traditional Chinese medicine group （ labor rate ） was significantly increased. Before treatment, compared with the blank control group, Western medicine group and traditional Chinese medicine group Th1/Th2 balance was significantly shifted to the Thl type, after treatment, the Western medicine group and traditional Chinese medicine group Th1/Th2 balance was significantly shifted to the Th2 type. Conclusion ： The serum levels of Th1/Th2 cytokines in patients with recurrent spontaneous abortion were significantly shifted to the Thl type, Bushen Guehong Decoction can effectively improve the clinical cure rate, and make the Th1/Th2 cell factor significantly shifted to the Th2 type.
