摘要
胃癌的发病率及死亡率近年来有所提升,文章结合多年的临床实践及近50年中医药防治胃癌的循证医学依据,概括总结了中医药在不同阶段防治胃癌的优势,癌前预防病变、术后促进康复、配合放化疗增效减毒;晚期患者提高生活质量,延长生存期等。同时,结合"种子和土壤"学说,以胃癌细胞和炎性免疫微环境为切入点对中医药治疗胃癌的多环节,多靶点的分子机制特点进行了揭示,为未来中医药进行胃癌的精准治疗提供一定的科学依据。
It is accepted that TCM can prevent precancerous lesions, promote postoperative recovery, reduce the toxicity of chemotherapy, enhance the antitumor effect of chemotherapy, alleviate tumor - induced clinical symptoms, prolong the survival time of advanced stage cancer patients and improve patients＇ quality of life （QOL）. This review summarizes TCM clinical practices in GC patients. Further more ,we illustrate multitargeted and bidirectional characteristics of TCM in treating GC. The molecular mechanisims of TCM in inhibiting GC invasion and metastasis probably are connected to modulating related biomarkers, such as regulating the secretion of immune suppressive factors （ IL - 6, TNF - α,TAMs, MDSCs） , decreasing CD4 ＋ , CD8 ＋ T cells apoptosis, improving immune surveillance capability of T cell and decreasing secretion of VEGF, to induce cell apoptosis, reverse trans- tbrmation of cancer stem cells, promote immune microenvironment. Hopely, discussion of the recent development and advancements in TCM will be helpful for providing novel insight into gastric cancer treatment.
