摘要
目的研究腹腔镜下阴式子宫切除术临床治疗方法和应用效果。方法以本院收治的子宫切除术患者120例作为研究对象,其中经腹子宫切除术36例(记为A组),阴式子宫全切术46例(记为B组),腹腔镜下阴式全子宫切除术38例(记为C组),观察3组患者的治疗效果及并发症情况。结果A组患者术中出血量、手术时间、肛门排气时间均显著高于其余2组,差异有统计学意义(P〈0.05),且C组除手术时间外各项数据均较B组显著降低(P〈0.05);A组患者并发症发生率和B组比较无显著差异(P〉0.05),但二者均较c组显著升高,且差异有统计学意义(P〈0.05)。结论选择合适病例行腹腔镜全子宫切除术可有效促进术后恢复。而对于子宫体积较大、有盆腹腔多次手术史或盆腔粘连严重者,应根据实际情况适时中转开腹治疗。
Objective To study on the clinical treatment method of laparoscopic vaginal total hysterectomy and effect evaluation. Methods One hundred and twenty cases of patients treated with excision of uterus in our hospital were as the research subjects,a- mong which there were thirty-six cases of patients treated with trans-abdominal hysterectomy (as the A group), forty-six cases of patients treated with vaginal total hysterectomy (as the B group) and thirty-eight cases treated with laparoscopic vaginal total hys-terectomy (as the C group). The treatment effect and complications were observed among the three groups. Results The amount of bleeding during operation,operation time, anal exhaust time in the A group were significantly higher than those in the other two groups,between which the differences were statistically significant (P〈0.0,5 ), and the data in the C group except for operation time in were significantly lower than those in the B group (P〈0.05). There were no significant differences in the incidence rates of cornplications between the A group and B group (P〉0.05),which were significantly higher than that in the C group,and the differences were statistically significant (P〈0.05), Conclusion Laparoscopic hysterectomy with appropriate disease can effectively pro-mote postoperative recovery. The patients with larger uterine volume, many cases of pelvic abdominal surgery or pelvic adhesion should be timely given conversion to open treatment according to the actual situation.
