摘要
目的比较精细化护理和常规护理对腹腔镜辅助下阴式子宫切除术(LAVH)患者术后疼痛的影响。方法选择2016年7月—2018年6月该科室行LAVH患者80例,将患者随机分为精细化护理组和常规护理组。常规护理组患者给予LAVH术后常规护理,精细化护理组在常规护理的基础上给予阴道护理、感染护理、内部出血护理、膀胱护理、粘连护理和并发症评估和预防。评价两组患者术后2、6、12、24 h疼痛强度和24 h内止痛剂的用量。结果精细化护理组术后2 h疼痛强度(4.602±1.429)显著低于常规护理组(6.325±1.985);精细化护理组术后24 h阿片类止痛剂的用量(3.269±11.425)mg显著低于常规护理组(13.041±17.562)mg,差异有统计学意义(t=2.509、3.477,P=0.026、0.025)。两组患者术后6、12、24 h疼痛强度和24 h内非甾体止痛剂用量比较,差异无统计学意义(t=0.215、0.159、0.365、0.432,P=0.198、0.958、0.155、0.659)。结论精细化护理能显著降低LAVH患者术后早期疼痛及镇痛药的用量。
Objective To compare the effects of refined nursing and routine nursing on postoperative pain in patients undergoing laparoscopic assisted vaginal hysterectomy(LAVH).Methods From July 2016 to June 2018,80 patients with LAVH were enrolled in the department.The patients were randomly divided into the refined nursing group and the routine nursing group.Patients in the routine care group were given routine care after LAVH.The refined nursing group was given vaginal care,infection care,internal bleeding care,bladder care,adhesion care and complications assessment and prevention on the basis of routine care.The pain intensity at 2,6,12,and 24 h after surgery and the amount of analgesic within 24 hours were evaluated.Results The pain intensity of the 2 h postoperative care group was(4.602±1.429),significantly lower than that of the conventional nursing group(6.325±1.985).The amount of opioid analgesic(3.269±11.425)mg was significantly lower in the refined nursing group 24 hours after surgery nursing group(13.041±17.562)mg,The difference was statistically significant(t=2.509,3.477,P=0.026,0.025).The pain intensity at 6,12,and 24 h after surgery and the amount of non-steroidal analgesics in the two groups.The difference was not statistically significant(t=0.215,0.159,0.365,0.432,P=0.198,0.958,0.155,0.659).Conclusion Fine nursing can significantly reduce the amount of early postoperative pain and analgesics in patients with LAVH.
