摘要
目的探讨联合检测血清中癌胚抗原(CEA)、糖类抗原125(CA125)、153(CA153)和199(CA199)对肺癌的诊断价值。方法收集131例肺癌患者作为实验组,115例肺部良性疾病患者作为对照组,采用酶免疫化学发光法测定患者血清中CEA、采用电化学发光法测定患者血清中CA125,CA153和CA199水平。结果实验组患者血清中CEA,CA125、CA199和CA153水平均显著高于对照组(P<0.05),CA125水平与对照组相比差异无统计学意义(P>0.05)。TNM分期中,III^IV期肺癌患者血清CEA,CA125和CA153的阳性率显著高于I^II期(P<0.05)。受试者工作特征曲线下面积(AUC)显示,四种肿瘤标志物中CEA诊断效能最高(0.75)。联合检测胸水中CEA,CA199和CA153优于其他指标,具有83.2%的灵敏度,93.0%的特异度和87.8%准确度。结论联合检测胸水中CEA,CA199和CA153更有助于提高肺癌诊断的灵敏度、特异度和准确度。
Objective To evaluate the diagnostic value of carcinoembryonic antigen （CEA）, cancer antigen CA125, CA153 and CA199 for patients with lung cancer. Methods A total of 131 patients with lung cancer and 115 patients with benign pulmonary lesions were collected, and the pleural effusion concentrations of CEA, CA125, CA199 and CA153 were measured by electro-chemiluminescence immunoassay. Results In pleural effusion, concentrations of CEA, CA153 and CA199 in lung cancer group were higher than that in the control group （P 〈 0.05 ）. Higher positive rates of CEA, CA199 and CA153 were found in patients with III- IV stage lung cancer than that in patients with I- II stage （ P 〈 0.05 ）. The area under the receiver operating characteristic curve （AUC） showed CA153 was a better tumor marker out of all four tumor markers with the highest diagnostic performance （0.75）. Combination of CEA, CA199 and CA153 in pleural effusion had 83.2% of sensitivity, 93.0% of specificity and 87.8% of accuracy, which gave much better performance than other makers tested alon. Conclusion The combination detection of tumor markers of CEA, CA199 and CA153 in pleural effusion improves the diagnostic sensitivity, specificity and accuracy for lung cancer.
