摘要
目的探讨超声弹性成像技术在甲状腺癌中的诊断价值。方法回顾性分析2011年7月至2013年7月内蒙古医科大学附属医院收治的72例甲状腺癌患者的临床资料,患者均先行常规二维超声检查,后行超声弹性成像检查,并采用5分法对甲状腺结节弹性图像进行评分。以病理结果为金标准,判断弹性超声成像在诊断甲状腺癌中的诊断学评价效能。结果 72例患者中共有88个甲状腺结节,其中良性结节为69个,占78.4%,恶性结节19个,占21.6%。以病理检查结果为金标准,超声弹性成像检测技术的灵敏度为68.6%,特异度为90.1%,高于二维超声的85.2%和48.6%。超声弹性成像的受试者工作特征曲线下面积(0.888)大于二维超声的曲线下面积(0.743)。结论超声弹性成像在甲状腺癌中具有较高的诊断价值。
Objective To investigate the diagnostic value of ultrasonic elasticity imaging in thyroid cancer.Methods Retrospective analysis of clinical data of 72 cases of thyroid cancer treated in Affiliated Hospital of Inner Mongolia Medical University from Jul.2011 to Jul.2013 was done,all patients were firstly given conventional two-dimensional ultrasound examination,and then ultrasonic elasticity imaging examina-tion ,and 5 points method was used to score the thyroid nodules elasticity image .With pathological findings as gold standard,the effectiveness of ultrasonic elasticity imaging in the diagnosis of thyroid cancer was evalua-ted.Results There were 88 thyroid nodules in the 72 patients,including 69 benign nodules,accounting for 78.4%,19 malignant nodules,accounting for 21.6%.With pathological findings as the gold standard,the sensitivity of ultrasonic elasticity imaging detection technology was 68.6%, specificity was 90.1%, higher than the two-dimensional ultrasound′s 85.2% and 48.6%;receiver operator characteristic curve area of ultrasonic elasticity imagingwas 0.888, greater than the area of two-dimensional ultrasound′s 0.743. Conclusion Ultrasonic elasticity imaging has high diagnostic value in thyroid cancer .
