摘要
目的探讨死亡相关蛋白激酶(death-associated protein kinase,DAPK)、人类RUNT相关转录因子3(human RUNT-related transcription factor 3,RUNX3)基因启动子区CpG岛在膀胱癌中异常甲基化状态及其在膀胱癌发生中的作用。方法采用实时荧光定量甲基化特异性PCR法检测76例原发性膀胱癌患者的膀胱癌组织标本(膀胱癌组)及39例泌尿系统结石患者的正常膀胱黏膜组织标本(对照组)的DAPK和RUNX3基因启动子区甲基化状态,比较2组DAPK和RUNX3基因启动子区甲基化发生率,分析其甲基化发生率与膀胱癌临床特征间关系。结果膀胱癌组DAPK基因和RUNX3基因启动子区甲基化发生率(64.5%、67.1%)高于对照组(43.6%、38.5%),差异均有统计学意义(P<0.05);膀胱癌组DAPK基因启动子区甲基化发生率在不同分化程度和病理分级上差异均有统计学意义(P<0.05),RUNX3基因启动子区甲基化发生率在肿瘤发生特征、分化程度、病理分级和肿瘤分期上差异均有统计学意义(P<0.05)。结论 DAPK和RUNX3基因启动子区CpG岛高度甲基化可能参与了膀胱癌的发生、发展过程,是膀胱癌组织的遗传学改变之一,对膀胱癌早期诊断、复发监测和预后判断有重要意义。
Objective To investigate the methylation status of CpG island of death-associated protein kinase(DAPK)and human RUNT-related transcription factor 3(RUNX3)gene promotors in patients with bladder cancer.Methods Realtime quantitative methylation-specific PCR was adopted to detect the promotor methylation status of DAPK and RUNX3 genes from the cancer tissue in 76 patients with primary bladder cancer(cancer group)and normal bladder mucosa tissue from 39 patients with urinary stone(control group).The methylation rates of DARK and RUNX3 gene promotors were compared between two groups to analyze the relationship between the methylation rates and the clinical characterstics of bladder cancer.Results The methylation rates of DAPK and RUNX3 genes were significantly higher in cancer group(64.5%,67.1%)than those in control group(43.6%,38.5%)(P〈0.05).There were significant differences in the methylation rates of DAPK gene among the differentiation degree and tumor pathological grades(P〈0.05),and in the methylation rates of RUNX3 gene among the different tumor characteristics,differentiation degrees,pathological grades and stages(P〈0.05).Conclusion Being agenetic change of bladder cancer,the methylation of CpG island of DAPK and RUNX3 genes may probably participate in the development and progression of bladder cancer,and play an important role in the early diagnosis,monitoring the recurrence and predicting the prognosis of bladder cancer.
