摘要
目的：探讨两性霉素B联合伏立康唑对不同免疫状态肺隐球菌病的临床观察.方法：选择2012年1月～2015年9月某院接诊的不同免疫状态肺隐球菌病76例患者进行研究,随机分为观察组和对照组.观察组38例患者给予两性霉素B联合伏立康唑进行治疗,对照组38例患者给予两性霉素B联合氟康唑进行治疗,对比分析两组患者的并发症、不良症状消失等情况.结果：治疗之后,观察组在丙氨酸氨基转移酶、门冬氨酸转移酶和肌酐上均要高于对照组（77.25±15.68vs33.47±12.89、36.65±11.21vs24.71±10.82、74.79±10.31umol/Lvs50.34±9.99umol/L）,两组比较具有统计学意义（P〈0.05）;对照组患者发热、头痛、呕吐、抽搐和意识性障碍等并发症上要显著高于观察组,对照组患者在发热消退与头痛减轻时间上均要明显高于观察组患者（3.56±1.20vs5.28±1.88、4.25±1.31vs6.03±1.44）min,两组比较具有统计学意义（P〈0.05）.结论：对不同免疫状态肺隐球菌病两性霉素B 联合伏立康唑进行治疗效果良好,是一种较好的治疗方案,值得在临床中广泛推广和应用.
Objective:To study clinical observation of Amphotericin B combined voriconazole on different immune status of pulmonary cryptococcosis.Methods:76patients with the immune status of the different pulmonary cryptococcosis treated in a hospital from January 2012 to September 2015 were researched,who were randomly divided into observation and control groups.38 patients in the observation group were treated by amphotericin B combined voriconazole and 38 patients in the control group were treated by amphotericin B combined with fluconazole.Then compare the complications and disappearance of adverse reaction of the two groups.Results:After treatment,the alanine aminotransferase and aspartate aminotransferase of the observation group were higher than those of the control group(77.25 ± 15.68 vs 33.47±12.89(4~50U/L),36.65±11.21 vs 24.71±10.82(5~40U/L,74.79±10.31 On vs 50.34±9.99(umol/L),which were statistically significant(P<0.05);the complications such as fever,headache,vomiting,convulsions and consciousness disorders of the control group were higher than those of the observation group.The disappearance time of fever and headache of the control group were longer than that of the observation group(3.56±1.20 vs 5.28±1.88、4.25±1.31vs6.03±1.44)min,which was statistically significant(P<0.05).Conclusion:Amphotericin B combined voriconazole treatment is good for different immune status of pulmonary cryptococcosis and is agood treatment plan,which should be popularized and applied in clinical practice.
