摘要
目的研究结核性胸膜炎运用结核感染T细胞斑点试验(T-SPOT.TB)的临床诊断评价,为临床诊治工作提供理论依据。方法选择2014年1月-2015年1月医院收治的50例患有结核性胸膜炎患者,分别对所选的患者实施外周血T-SPOT.TB、胸腔积液腺苷脱氨酶(ADA)、血清结核抗体等指标进行测定,同时比较3项指标对结核性胸膜炎实施诊断的灵敏度,数据采用SPSS 14.0软件进行统计分析。结果外周血T-SPOT.TB、胸腔积液ADA、血清结核抗体的灵敏度分别为92.6%、95.5%、36.4%,特异度分别为92.3%、75.0%、78.6%;阳性预测值分别为96.2%、91.7%、57.1%;阴性预测值分别为85.7%、85.7%、35.3%;外周血T-SPOT.TB特异度明显优于ADA、血清结核抗体两种方法的指标,差异有统计学意义(P<0.05)。结论结核性胸膜炎通过外周血T-SPOT.TB方法实施诊断的特异度以及灵敏度均优于其他两种方法,而且优势较为明显,所采取的操作方式与胸腔积液ADA检测方法基本一致,但是明显优势在于其特异度,价值明显,值得在临床上推广运用。
OBJECTIVE To study the clinical diagnostic evaluation of tuberculosis infection T cell spot test（TSPOT.TB）for tuberculous pleurisy,to provide a theoretical basis for clinical diagnosis and treatment.METHODS A total of 50 cases of patients admitted in our hospital suffering from tuberculous pleurisy in Jan.2014 to Jan.2015 were selected.Peripheral T-SPOT.TB,pleural fluid adenosine deaminase（ADA）,and serum antibodies were respectively conducted for patients.Sensitivity diagnosis of the three indexes on the implementation of tuberculous pleurisy were compared.The results were statistically analyzed by software SPSS 14.0.RESULTS The sensitivity of peripheral blood T-SPOT.TB,ADA,tuberculosis antibody in serum were respectively 92.6%,95.5% and 36.4%;the specificity of them were respectively 92.3%,75.0% and 78.6%;the positive predictive values of them were respectively 96.2%,91.7% and 57.1%;and the negative predictive values were respectively85.7%,85.7% and 35.3%.The specificity of peripheral T-SPOT.TB was superior to the indexes of ADA and tuberculosis antibody,and the difference was significant（P〈0.05）.CONCLUSIONThe diagnostic specificity and sensitivity of tuberculous pleurisy by the method of peripheral blood T-SPOT.TB are much better than that of the other two methods.Its operation methods are consistent with pleural effusion ADA detection method,but its distinct advantages are in its specificity,great value,and it is worth clinical use.
