冠心病患者超敏C反应蛋白及平均血小板体积的预测价值 被引量:3

The predictive value of hypersensitive C-reactive protein and mean platelet volume in coronary heart disease patients

摘要 目的检测对冠心病患者的超敏C反应蛋白(hs-CRP)及平均血小板体积(MPV),分析其对冠心病的预测价值。方法选取2012年1月至2015年6月于佛山市顺德区第一人民医院经冠状动脉造影确诊的冠心病患者312例,同期选择健康体检者199人作为对照组,检测血常规、血脂及hs-CRP水平。结果冠心病组的总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、hs-CRP、MPV与对照组比较差异有统计学意义(P〈0.05),而年龄、体重指数(BMI)、高密度脂蛋白胆固醇(HDL-C)、甘油三脂(TG)差异无统计学意义(P〉0.05)。经Logistic回归分析,TC、LDL-C、hsCRP、MPV均是冠心病发病的独立危险因素(P〈0.05);Hs-CRP与MPV呈正相关(r=0.306,P〈0.05)。Hs-CRP与MPV的受试者操作特征曲线分析,hs-CRP预测冠心病的最佳截点为3.83 mg/L,ROC曲线下面积为0.910(95%CI:0.887~0.934,P〈0.05),特异性为82.91%,敏感度为84.29%;MPV在预测冠心病的最佳截点为9.37 f L,ROC曲线下面积为0.748(95%CI:0.706~0.790,P〈0.05),特异性为81.41%,敏感度为54.50%。结论检测hs-CRP、MPV是预测冠心病简单、实用的方法,hs-CRP对冠心病预测的特异性和敏感度较MPV高。 Objective To detect the value of hypersensitive C-reactive protein(hs-CRP) and mean platelet volume(MPV)in coronary heart disease(CHD) patients and analyze the predictive and cut-off value of hs-CRP and MPV for CHD.Methods 312 cases of CHD patients confirmed by coronary angiography were selected asobservation group,and 199 healthy people were as the control group.Routine blood test was performed.Blood lipid and hypersensitive C-reactive protein were mesured. Results Compared with control group,total cholesterol(TC),low density lipoprotein-cholesterol(LDL-C),hs-CRP,MPV of CHD group were different statistically(P 〈0.05). While age,body mass index(BMI),high density lipoprotein-cholesterol(HDL-C) and triglycerides(TG) had no significant differences between two groups(P〉0.05).Logistic regression analysis revealed that TC,LDL-C,hs-CRP and MPV were the independent risk factors for CHD(P〈0.05). Hs-CRP and MPV were positively correlated(r=0.306,P〈0.05). Receiveroperating characteristic(ROC) curve analysis indicated the best point of hs-CRP to predict CHD was 3.83 mg/L,the area under the ROC curve was 0.910(95% CI :0.887~0.934,P〈0.05); the best point of MPV predicting CHD was 9.37 fl,the area under the ROC curve was 0.748(95% CI : 0.706 ~0.790,P 〈0.05). Conclusion Detection of hs-CRP and MPV is a simple,practical way to predict CHD;the specificity and sensitivity of hs-CRP for the prediction of CHD were higher than MPV.
