老年冠心病患者血浆脑钠肽的检测及其临床意义

摘要 目的观察不同临床类型冠心病患者的血浆脑钠肽(BNP)浓度的差异及其与C反应蛋白(CRP)的相关性,并探讨其临床意义。方法88例冠心病患者分为:急性心肌梗死(AMI)22例,不稳定型心绞痛(UAP)36例,稳定型心绞痛(SAP)30例;30例健康者作为对照组。检测各组的血浆BNP浓度和CRP浓度,比较不同组别BNP的差异以及BNP与CRP的关系。结果AMI、UAP、SAP和对照组血浆BNP浓度分别为(560·23±122·13)pg/mL,(166·33±56·17)pg/mL,(48·16±26·43)pg/mL和(42·35±24·11)pg/mL,AMl组平均BNP明湿高于UAP组(P<0·001),UAP组平均BNP明显高于SAP组和对照组(P<0·001),SAP组和对照组BNP无明显差异(P>0·05)。AMI、UAP、SAP和对照组血浆CRP浓度分别为(23·4±11·3)mg/L,(21·3±9·1)mg/L,(5·8±3·1)mg/L和(5·5±2·9)mg/L;AMI组和UAP组平均CRP均明显高于SAP组和对照组(P<0·001,P<0·001),SAP组和对照组CRP无明H差异(P>0·05)。BNP与CRP在AMI和UAP组均相关(r=0·61P<0·01,r=0·56P<0·01)。结论不同临床类型冠心病患者的血浆BNP浓度存在差异,BNP和CRP均可作为冠心病临床危险分层的指标。 Objective To explore the relationship between plasmlevel of B-Py'pe natriuretic peptide(BNP) and clinical phenotypes of coronary bean disease(CHD) and the correlation between BNP and CRP. Methods 88 patients with CHD were enrolled in this study and 30 healthy persons were selected as control. Clinical phenotypes of CHD included acute myocardial infarction(AMl),unstable angina pectoris(UAP) and stable angina pectoris(SAP). Plasma levels of BNP and CRP were detected at 16 hours after the appearance of symptoms in AMI or on admission in other groups and its clinical significances were analyzed. Correlation between BNP and CRP which was detected at the same time with BNP was also analyzed in AMI group and UAP group.Results Plasma levels of BNP were 560.23±122.13pg/mL, 166.33±56.17pg/mL,48.16±26.43pg/mL and 42.35±24.11 pg/mL in the patients with AMI,UAP, SAP and control respectively. The level of BNP was significantly higher in AMI than that in other three groups(P<0.001, P<0.001,P<0.001); The level of BNP was significantly higher in UAP than that in SAP or control(P<0.001, P<0.001); there were no significant difference in BNP level between SAP and control(P>0.05). The levels of CRP were significantly higher in AMI and UAP than that in SAP or control respectively(P<0.001, P<0.001); there were no significant difference in CRP level between SAP and control(P>0.05). BNP level were correlated with CRP in AMI and UAP(r=0.61 P<0.01 ,r-4).56/'<0.01 respeetively).Conclusion Plasma level of BNP are associated with the clinical phenotypes of CHD and it may reflect the risk degree of clinical phenols'pcs of CHD, likely as CRP.
