摘要
目的探讨血清抗磷脂酶A2受体(anti-phospholipase A2receptor,PLA2R)抗体诊断特发性膜性肾病(idiopathic membranous nephropathy,IMN)的价值。方法 76例肾脏病患者,肾穿刺活检组织病理证实IMN患者31例,非IMN患者45例。采用间接免疫荧光法检测其抗PLA2R抗体水平;以肾穿刺活检组织病理检查结果为金标准,计算抗PLA2R抗体诊断IMN灵敏度、特异度、阳性预测值、阴性预测值、阳性似然比、阴性似然比、Kappa系数。结果 31例IMN患者中,抗PLA2R抗体阳性28例、阴性3例,45例非IMN患者抗PLA2R抗体均为阴性;与肾穿刺活检组织病理进行对照,抗PLA2R抗体诊断IMN的灵敏度为90.32%,特异度为100.00%,假阴性率为9.68%,假阳性率为0,阳性预测值为100.00%,阴性预测值为93.75%,阳性似然比为0,阴性似然比为9.68%,Kappa系数为0.917。结论抗PLA2R抗体作为IMN的特异性标志物,可用于IMN的诊断和鉴别诊断。
Objective To explore the value of anti-phospholipase A2receptor（anti-PLA2R）antibody to the diagnosis of idiopathic membranous nephropathy（IMN）.Methods The histopathological results from 76 patients with kidney disease confirmed 31 patients with IMN and 45 patients with no IMN.The level of anti-PLA2 Rantibody was detected by indirect immunofluorescence method.Taking the histopathological results as the golden standard,the sensitivity,specificity,positive predictive value,negative predictive value,positive likelihood ratio,negative likelihood ratio and Kappa coefficient of anti-PLA2 Rantibody were calculated.Results In 31 patients with IMN,positive anti-PLA2 Rantibody was found in 28 patients and negative in 3patients.And the anti-PLA2 Rantibody showed negative in all 45 patients with no IMN.The sensitivity of the anti-PLA2 Rantibody for IMN was 90.32%,specificity was 100.00%,false negative rate was 9.68%,false positive rate was 0,positive predictive value was 100%,negative predictive value was 93.75%,positive likelihood ratio was 0,negative likelihood ratio was 9.68%,and Kappa coefficient was 0.917.Conclusion AntiPLA2 Rantibody can be used as a specific indicator for IMN and can be used in the differential diagnosis of IMN.
