摘要
目的分析磷脂酶A2受体(phospholipase A2 receptor,PLA2R)水平的诊断价值,并对免疫荧光测定(indirect immunofluorescence assay,IIF)法及酶联免疫吸附法(enzyme-linked immunoSorbent assay, ELISA)测定PLA2R水平的一致性进行分析。方法将2018年1月至12月于内蒙古医科大学附属医院风湿免疫科行肾组织活检并确诊的96例肾病患者纳入研究,根据其病理结果分为膜性肾病组与非膜性肾病组,纳入同期健康体检人员为对照组,收集患者血清,比较患者肌酐、尿素等相关指标,分别采用IIF及ELISA测定患者PLA2R水平,对两种检测方法的相关性及一致性进行分析。结果采用ELISA法测得肾病患者的PLA2R明显高于对照组,且膜性肾病组患者的PLA2R水平明显高于非膜性肾病组,其中ELISA法及IIF法均检测出膜性肾病组有31例阳性患者,而非膜性肾病组及对照组则阳性例数为0(均P<0.05);采用ELISA法与IIF法检测PLA2R的一致率为94.87%(37/39)(Kappa=0.897,P=0.000),且两种检测方式结果明显相关(ρ=0.900,P=0.000);但ROC曲线示ELISA法的AUC为0.998明显高于IIF法、白蛋白、总蛋白、TC及TG的诊断效能(均P<0.05)。结论 IIF与ELISA测PLA2R水平具有较高的一致性及相关性,但ELISA在诊断膜性肾病的诊断效能更高,且操作简单,可供临床应用。
Objective To evaluate the diagnostic value of PLA2R level, and analyze the consistency of PLA2R levels measured by indirect immunofluorescence(IIF) assayand enzyme immunosorbent assay(ELISA). Methods A total of 96 patients with renal disease who underwent renal tissue biopsy and confirmed in our hospital from January 2018 to December 2018 were included in the study. According to their pathological results, they were divided into membranous nephropathy group and non-membranous nephropathy group.The health check-up staff in the same period were enrolled as the control group. The patients’ serum was collected, and the IIF and ELISA were used to evaluate patients’ PLA2R levels,and the correlation and consistency between the two detection methods were analyzed. Results PLA2R measured by ELISA in patients with kidney disease was significantly higher than that in the control group,and the level of PLA2R in patients with membranous nephropathy was significantly higher than that in non-membrane nephropathy. Thirty-one positive patients were detected by both IIF assay and ELISA in the membranous nephropathy group, the number of positive cases in non-membrane nephropathy group and control group was zero(both P<0.05). The consistency of PLA2R detected by ELISA and IIF assay was 94.87%(37/39)(Kappa=0.897,P=0.000). The results of the two methods were significantly correlated(ρ=0.900,P=0.000). However, the ROC curve showed that the AUC of ELISA was 0.998, which was significantly higher than that of IIF assay, albumin, total protein, TC and TG(all P<0.05). Conclusions There is a high consistency and correlation between the IIF and ELISA assay in measuring PLA2R level.However, ELISA has higher diagnostic efficiency in diagnosis of membranous nephropathy,which is easy to operate and can be used in clinical applications.
