摘要
目的探讨后路椎板部分切除经椎间孔减压、椎体复位、椎间植骨融合、内固定治疗下腰椎滑脱症的临床疗效。方法采用回顾性分析,整群选取该院2009年3月—2014年2月间手术治疗下腰椎滑脱症患者65例,随机分为观察组和对照组,观察组33例患者给予经椎间孔腰椎椎体间融合治疗,对照组32例患者给予后路腰椎椎体间融合治疗,比较2组治疗效果。结果观察组患者平均手术时间(150.4±15.1)min,明显短于对照组的(170.2±20.5)min(P<0.05);观察组患者平均术中出血量(320.5±50.3)m L,明显低于对照组的(480.2±44.6)m L(P<0.05);观察组患者的平均住院时间为(8.8±1.8)d,明显短于对照组的(14.5±1.5)d(P<0.05);观察组患者术后并发症的发生率(3.03%),明显低于对照组(9.37%)(P<0.05);2组患者术后VAS评分均较术前降低(P<0.05);而术后2组间比较:VAS评分、复位率及融合率差异无统计学意义(P>0.05)。结论经椎间孔减压、椎间融合、复位内固定治疗下腰椎滑脱症神经管减压彻底,复位满意,椎间植骨融合满意,创伤小,并发症少,疗效确切、安全、可靠。
Objective To investigate the clinical effects of intervertebral foramen decompression, vertebrae reduction, inter-body spinal fusion and internal fixation in the treatment of lower lumbar spondylolisthesis. Methods 65 patients with lower lumbar spondylolisthesis who underwent surgical treatment from March 2009 to February 2014 were retrospectively ana-lyzed. They were randomly divided into observation group and control group. 33 patients in the observation group were treated with transforaminal lumbar interbody fusion, while the other 32 in the control group were treated with posterior lum-bar interbody fusion. The curative effect was compared between the two groups. Results The mean operative time [（150.4 ± 15.1 ） min vs（170.2±20.5 ） min, P〈0.05] was significantly shorter in the observation group than in the control group;the av-erage blood loss[（320.5±50.3） mL vs（480.2±44.6）mL, P〈0.05] was obviously lower in the observation group than in the con-trol group; the even lengthy of hospital stay [（8.8±1.8 ）d vs （14.5±1.5）d, P〈0.05] was significantly shorter in the observation group than in the control group;the postoperative complication rate （3.03%vs 9.37%, P〈0.05） was significantly lower in the observation group than in the control group. The VAS score after operation was lower than that before operation in both of the two groups;postoperative inter-group comparison showed that no statistically significant difference can be found in VAS score, reduction rate, fusion rate terms between the two groups （P〉0.05）. Conclusion Intervertebral foramen decompression, intervertebral fusion, reduction and internal fixation in treating lower lumbar spondylolisthesis can get full decompression, satisfactory reduction and bone graft fusion with little trauma, fewer complication, safety, and exact and reliable efficacy.
