Depuy pedicle screw system combined with posterior circumferential fusion for the treatment of lumbar spondylolisthesis

摘要 [目的]探讨Depuy椎弓根钉棒系统联合后路环形植骨融合治疗腰椎滑脱症的临床疗效。[方法]2007年1月~2010年1月,58例腰椎滑脱症患者均采用Depuy椎弓根钉棒系统联合后路环形植骨融合进行治疗,分别比较术前、术后1周和随访末次随访椎间隙高度、滑脱角、Boxall指数及腰椎生理前凸角。腰椎功能改善情况采用日本骨科协会评分标准(Japanese Orthopaedics Association,JOA)评估。[结果]58例患者术后均获24~60个月随访。根据Nakai评分标准,优49例,良8例,可1例,优良率达98.3%。术前JOA评分为(13.21±1.41)分,术后1周JOA评分为(24.27±1.23)分,末次随访JOA评分为(25.35±1.31)分。术前与术后1周、末次随访比较差异有统计学意义(P<0.05)。本组患者3~12个月内均获得骨性融合,无Cage融合器移位或沉陷,椎弓根钉棒松动、移位、断裂,甚至滑脱复发等并发症发生。术后1周椎间隙高度、滑脱角、Boxall指数及腰椎生理前凸角与术前比较差异有统计学意义(P<0.05);而术后1周与末次随访各指标比较差异无统计学意义(P>0.05)。[结论]Depuy椎弓根钉棒系统联合后路环形植骨融合治疗腰椎滑脱症,固定牢靠,复位满意,融合率高,是治疗腰椎滑脱症较为理想的方法。 [ Objective] To evaluate the clinical effect of the treatment of lumbar spondylolisthesis by Depuy pedicle screw system combined with posterior circumferential fusion. [ Methods] From January 2008 to January 2011, 58 cases with lumbar spondylolisthesis were treated with Depuy pedicle screw system combined with posterior circumferential fusion. Height of the in- tervetebral space, the spondylisthesis angle, Boxall index and lumbar lordosis angle were compared respectively before opera- tion, and at 1 week and final follow - up after operation. Japanese Orthopaedics Association (JOA) score system was used to e- valuate the lumbar functional improvement. [ Results ] Fifty - eight cases were followed up for 24 - 60 months after operation. According to Nakai' s Standard, the results were excellent in 49 cases, good in 8 cases, and fair in 1 case, the excellent and good results rate was 98.3%. The mean scores of JOA was ( 13.21 ± 1.41 ) at preoperation, (24. 27 ± 1.23) at 1 week posto- peration, (25.35 ± 1.31 ) at final follow - up. The JOA score showed statistical significance ( P 〈0.05) between preoperation and postoperation. All of the embedded osseous were fused, the fusing time was from 3 to 12 months. There was no complica- tions of migration or subsidence of the whole Cage, and fixation loosening, migration, breakage of screw and rob, even slip- page recurrence. There were statistically significant differences in height of the intervetebral space, the spondylisthesis angle, Boxall index and lumbar lordosis angle between preoperation and 1 week after operation ( P 〈 0. 05 ) , and the difference was not significant between 1 week postoperation and final follow - up (P 〉 0. 05) . [ Conclusion] Using Depuy pedicle screw system combined with posterior circumferential fusion for the treatment of lumbar spondylolisthesis provides strong fixation, good reduc- tion and high rate of graft fusion , which is an ideal method of the treatment of lumbar spondylolisthesis.
