腰椎滑脱患者脊柱-骨盆参数改变的研究进展 被引量:14

摘要 近些年来,腰椎滑脱患者的脊柱力线重建过程中的矢状位平衡问题越来越受到关注。评价脊柱-骨盆矢状位平衡的参数有骨盆投射角(pelvic incidence,PI)、骶骨倾斜角(sacral slope,SS)、骨盆倾斜角(pelvic tilt,PT)、胸椎后凸角(thoracic kyphosis,TK)、腰椎前凸角(lumbar lordosis,LL)[1~3],其中PI为固定解剖参数,不随人体的体位改变,PT、SS为体位相关性参数,一般取站立位的侧位片测量。腰椎滑脱发生后,人体为了维持站立姿势产生了一系列代偿机制,脊柱-骨盆平衡序列发生改变,相关研究[4~7]发现,腰椎滑脱的发生、进展、转归与矢状位平衡参数密切相关,同时腰椎滑脱患者的矢状位平衡参数数值对手术方式的选择具有指导意义,过去大多数学者认为手术的基本原则是:植骨融合加相应的内固定,而在是否实行复位这个问题上存在有较大争议[8、9]。笔者结合国内外文献,就脊柱-骨盆矢状位平衡参数在腰椎滑脱治疗中的意义综述如下。
