摘要
目的:研究结直肠癌淋巴结转移的相关影响因素。方法:收集2013年1月至2014年6月行根治性手术的全部结直肠癌临床病理资料,分析其对结直肠癌淋巴结转移的影响因素。结果:入组结直肠癌的淋巴结转移率为42%,单因素分析结果显示肿瘤大体(P=0.006)、组织类型(P=0.031)、分化程度(P=0.014)、浸润深度(P=0.000)、脉管内癌栓(P=0.000)及神经束膜受侵(P=0.000)与淋巴结转移密切相关。多因素分析结果显示肿瘤大体、浸润深度、脉管内癌栓是影响淋巴结转移的独立因素。结论:肿瘤大体、浸润深度、脉管内癌栓是影响淋巴结转移的独立因素,其中脉管内癌栓是淋巴结转移发生最重要的危险因素。
Objective： To investigate the clinicopathological factors related with the lymph node metastasis in patients with colorectal cancer. Methods： The clinicopathological data of 100 patients with colorectal cancer who underwent surgical resection in the Department of General Surgery, the Ganzhou Affiliated Hospital of Nanchang Medical University from January 2013 to July 2014 was retrospectively analyzed. The associations between Lymph node metastasis and chnicopathological factors were evaluated by Chi· quare test and Logistic regression analysis （SPSS 17.0 for windows）. Results： The rate of lymph node metastasis in colorectal cancer was 42.0%. Univariate analysis data showed that lymph node metastasis was related to their tumor gross type （P = 0. 006 ）, histological type （ P = 0.031 ）, tumor differentiation （ P = 0.014）, depth of invasion （P = 0. 000）, lymphovascular tumor thrombus （P = 0. 000） and perineural invasion （P = 0.000）. Multivariate analysis results demonstrated that tumor gross type, depth of invasion, lymphovascular tumor thrombus were independent factors affecting lymph node metastasis. Conclusion： Tumor gross type, depth of invasion, and lymphovascular tumor thrombus were independent factors related with lymph node metastasis in patients with colorectal cancer, especially, the lymphovascular tumor thrombus was the important risk factor of lymph node metastasis in colorectal cancer.
