妊娠期糖尿病患者血浆尿酸高表达与胰岛素抵抗的临床意义探讨 被引量:3

Clinical significance of high expression of plasma uric acid and insulin resistance in patients with gestational diabetes mellitus

摘要 目的 探讨妊娠期糖尿病(GDM)患者血浆尿酸(UA)高表达与胰岛素抵抗(IR)的相关性。方法 选取2012年5月~2013年12月在该院接受治疗的40例GDM患者作为实验组,另选取同时期在该院接受检查的40例妊娠期健康妇女为对照组。按照UA水平将实验组分为高尿酸组和尿酸正常组。比较各组空腹血糖(FPG)、空腹胰岛素(FINS)、UA水平和胰岛素抵抗指数(HOMA-IR),并对实验组患者的UA水平与FINS、HOMA-IR进行相关性分析。结果 实验组的FPG、FINS、UA及HOMA-IR均显著高于对照组(P〈0.05);高尿酸组患者FPG、FINS、UA与HOMA-IR水平均显著高于尿酸正常组(P〈0.05);实验组患者的UA和FINS、HOMA-IR均呈正相关(P〈0.05)。结论 GDM患者UA高表达与IR程度呈明显正相关性,这为临床上采用检测UA水平判断GDM患者IR程度提供了一定的科学依据。 Objective To explore the correlation between high expression of plasma uric acid (UA) and insulin resistance (IR) in patients with gestational diabetes mellitus (GDM) . Methods Forty GDM patients treated in the hospital from May 2012 to December 2013 were selected as experimental group, forty healthy pregnant women were selected from the hospital during the same period as control group. The cases in experimental group were divided into high UA group and normal UA group according to UA levels. The levels of fasting plasma glucose ( FPG), fasting insulin ( FINS), UA, and HOMA-IR were compared among the three groups, the correlations between UA level and FINS, HOMA-IR in experimental group were analyzed. Results FPG, FINS, UA levels, and HOMA-IR in experimental group were sta- tistically significantly higher than those in control group (P〈0. 05 ) ; FPG, FINS, UA levels, and HOMA-IR in high UA group were statisti- cally significantly higher than those in normal UA group (P〈0. 05 ) ; in experimental group, UA was positively correlated with FINS and HO-MA-IR (P〈O. 05) . Conclusion High expression of UA in patients with GDM is significantly positively correlated with IR degree, which provides a certain scientific basis for detecting UA levels to estimate the extent of IR in patients with GDM.
