摘要
目的评价宫颈液基细胞学检查(TCT)联合宫颈细胞DNA倍体定量分析在宫颈癌筛查中的意义。方法对2013年10月到2014年12月来我院就诊的2250名女性采用TCT联合宫颈细胞DNA倍体分析的筛查,宫颈液基细胞学制片2张,一张行巴氏染色做常规细胞学检查,另一张用硫堇-Feulgen染色后,采用DNA定量分析系统对细胞核DNA含量进行定量分析。以病理检查作为金标准。结果 12250例宫颈脱落细胞样本中,TCT阳性137例,阳性率6.09%;DNA倍体分析阳性164例,阳性率7.29%;TCT联合DNA倍体分析阳性192例,阳性率8.53%;2TCT阳性者进行组织活检,符合率54.01%(74/137);DNA倍体分析阳性者进行组织活检,符合率48.78%(80/164);TCT联合DNA倍体分析检测阳性,符合率70.31%(135/192)。结论宫颈液基细胞学检查联合DNA倍体定量分析,能提高诊断敏感性和符合率。
OBJECTIVE To evaluate the significance of liquid-based cervical cytology combined with quantita- tive analysis of cervical cellular DNA ploidy in cervical cancer screening. METHODS Screening of cervical cancer was performed by liquid-based cytology combined with quantitative analysis of cellular DNA ploidy in 2250 women who were in our hospital for visit from October 2013 to December 2014. The samples were taken by cervix brush and prepared for 2 slides ： one was stained by pap staining and read by cytologist. The other was stained with Thionin- Feulgen staining and test by DNA images cytometry. RESULTS （1）The TCT positive results were observed in 137 case （6.09%）. The DNA positive results were observed in 164 case （7.29%）. The TCT ＋ DNA positive results were observed in 192 case（8.53% ）. （2）By colposcopy biopsy, the positive coincidence rate of the TCT positive was 54. 01% （74/137）, the positive coincidence of The DNA positive was 48.78% （ 80/164 ）. the positive conineidence of the TCT ＋ DNA positive was 70. 31% （135/192）. CONCLUSION The liquid-based cervical cytology combined with quantitative analysis of cervical cellular DNA pIoidy could improve the sensitivity and coincidence rate.
