摘要
目的探讨羟考酮复合丙泊酚用于宫颈锥切术的麻醉效果。方法 80例择期行宫颈锥切术患者,随机分为两组,每组40例。两组均实施非气管插管静脉全麻,A组予芬太尼(1 mg·kg-1)复合丙泊酚麻醉,B组予羟考酮(0.1 mg·kg-1)复合丙泊酚麻醉,丙泊酚负荷量1.5 mg·kg-1,继之5 mg·kg-1·h-1维持麻醉。观察两组丙泊酚用量、苏醒时间、术后10 min时视觉模拟量表评分(VAS)和Ramsay镇静评分、不良反应发生情况等。结果两组手术时间、丙泊酚用量无显著差异(P>0.05),B组苏醒时间长于A组(P<0.05)。B组术后Ramsay镇静评分和苏醒时心率均低于A组(P<0.05),虽VAS>6分的发生率低于B组,但无显著差异(P>0.05)。A组不良反应发生率42%(17/40),高于B组(7/40,18%),差异显著(P<0.05)。结论羟考酮复合丙泊酚用于宫颈锥切术麻醉与芬太尼复合丙泊酚同样有效,但苏醒时患者血流动力学更平稳,镇静程度更佳,且不良反应发生率更低。
AIM To investigate the anesthetic effect of oxycodone combined with propofol in cervical conization. METHODS Eighty patients scheduled for cervical conization were randomly divided into two groups,40 patients in each. All the patients received general anesthesia without tracheal intubation. In addition, the patients in the group A received fentanyl 1 mg·kg-1+ propofol(loading dose 1.5 mg·kg-1, maintenance dose5 mg·kg-1·h-1), while the patients in the group B received oxycodone 0.1 mg·kg-1+ propofol. The amount of propofol, recovery time, VAS and Ramsay score after the operation, the incidence of adverse reactions etc.were observed. RESULTS There were no significant difference in surgical time and amount of propofol between group A and group B(P 〉 0.05). Recovery time in the group B was longer than that in the group A(P 〈0.05). Compared with the group A, the postoperative Ramsay score, heart rate during awakening were markedly reduced(P 〈 0.05). The percentage of VAS 〉 6 in the group B was lower than of that in the group A, but with no significant difference( P 〉 0.05). The incidence rate of adverse reactions of group A was 42%( 17/40),higher than that of the group B( 7/40, 18%, P 〈 0.05). CONCLUSION Oxycodone combined with propofol for anesthesia in cervical conization is as effective as fentanyl combined with propofol, with superior hemodynamic, better sedation effects, and less adverse reactions.
