摘要
目的综合评价血管紧张素转化酶(angiotensin converting enzyme,ACE)I/D基因多态性与中国汉族人群特发性肺间质纤维化(idiopathic pulmonary fibrosis,IPF)关系。方法应用计算机检索PubMed、Cochrane Library、中国学术期刊全文数据库、维普数据库和万方数据库,按纳入、排除标准选择纳入ACE基因I/D多态性与IPF关系的病例对照研究,评价纳入研究质量,并采用Stata 12.O进行定量分析。结果共纳入4个符合条件的病例对照研究,其中IPF组310例,对照组364例。Meta分析结果显示,所有模型中未见统计异质性,故均采用固定效应模型。等位基因(D VS I)、共显性模型(DDVSⅡ)及隐性模型(DD VS DI+Ⅱ)合并OR分别为[OR=1.701,95%CI(1.365,2.121)]、[OR=2.086,95%CI(1.403,3.102)]及[OR-2.544,95%CI(1.826~3.544)](P值均〈O.05)。结论ACE基因I/D多态性与中国汉族人群IPF的发病有密切关系,D等位基因和DD基因型是IPF的危险因素。但由于纳入文献数量较少,还需大样本、多中心研究进一步验证。
Objective To evaluate the relationship of angiotensin-converting enzyme (ACE)I/Dgene polymorphism with idiopathic pulmonary fibrosis (IPF). Methods We electronically searched PubMed, Cochrane Library, China Academic Journals Full-text database, VIP Database, Wanfang database,and collected the ACE I/D gene polymorphism and IPF relationship between case-control study among the Han Chinese population, and assessed the quality of the included studies. The data were analyzed by Stata 12.0 software. Results We included 4 articles,including 310 cases in IPF group and 364 cases in the control group. Meta-analysis results showed that there was no statistical heterogeneity in all models,therefore the fixed effects model were adopted. The combined OR in allele (D vs I), co-dominant model (DD vs II) and recessive model (DD vs DI + Ⅱ) were [ OR =1. 701,95%CI (1. 365,2. 121)], [OR =2.086,95% CI (1.403,3.102)],[OR =2.544,95% CI (1. 826~3. 544)] (all P 〈0.05) respectively. Conclusions The ACE gene I/D polymorphism is closely related to the pathogenesis of IPF in Chinese Han population,D allele and DD genotype is a risk factor for IPF. However,due to the small number of the researches included,it is also need a large sample, multi-center study to further verify.
