摘要
目的:通过观察和胃降逆法对反流性食管炎患者食管黏膜环氧合酶-2(COX-2)、E-钙黏蛋白(E-cadherin)表达的影响,探讨和胃降逆法治疗反流性食管炎的作用机理。方法:选择120例反流性食管炎患者,分为中药治疗组、西药治疗组;中药治疗组以和胃降逆法治疗,选用和胃降逆方口服;西药治疗组以奥美拉唑胶囊治疗;治疗8周。患者治疗前后均行胃镜检查及食管黏膜病理活检,免疫组化法测定食管黏膜COX-2、E-cadherin的表达。结果:和胃降逆法能明显改善反流性食管炎患者的组织病理学积分,能明显降低反流性食管炎患者食管黏膜组织COX-2的表达、增强黏膜组织E-cadherin表达。结论:降低食管黏膜组织COX-2表达、升高黏膜组织E-cadherin表达是和胃降逆法治疗反流性食管炎的作用机理之一。
Objective: To observe the effects of stomach - regulation and qi - lowering therapy on the ex- pressions of COX - 2 and E - cadherin in esophageal mucosa of patients with reflux esophagitis, and explore the mechanism. Methods :120 patients with reflux esophagitis were divided into two groups:Chinese medicine group and west medicine group. The Chinese medicine group was treated with the stomach - regulationg and qi - lower- ing therapy,while the west medicine group was treated with omeprazole capsule, they were treated for 8 weeks. All patients had examination of gastroscope and biopsy of esophageal mucosa before and after treatment. To observe the clinical therapeutic effect and esophageal mucosa pathological change and expressions of COX -2 and E - cadherin in esophageal mucosa were assayed by immunohistoehemistry. Results: Stomach - regulating and qi - lowering therapy could ameliorate the pathology change of esophageal mucosa tissue of reflux esophagitis, and sig- nificantly decrease the expression of COX - 2 and increase the expression of E - cadherm in esophageal mucosa of reflux esophagitis. Conclusion: Stomach - regulating and qi - lowering therapy could significantly decrease the expression of COX - 2 and increase the expression of E - cadherin in esophageal mucosa of patients with reflux esophagitis, and it may be one of the mechanisms of treating reflux esophagitis.
