摘要
目的:通过观察大鼠腹腔灌注5%Na HCO3后CO2气腹对腹腔液p H及TNF-α的影响,探讨其效果及机理,为临床腹腔镜手术治疗恶性肿瘤提供理论依据。方法:选取健康Wistar大鼠30只,分为3组。实验组:以10 ml 5%Na HCO3溶液灌洗腹腔,建立CO2气腹;气腹组:维持动态CO2气腹90 min;对照组:仅行单纯麻醉,不建立气腹。30 min时,实验组于腹腔内注入10 ml生理盐水灌洗,气腹组、对照组注入20 ml。分别于30 min、60 min、90 min、24 h、48 h、72 h取腹腔液样本,测定各组腹腔液p H值,酶联免疫吸附试验法测定TNF-α含量。结果:30 min、60 min、90 min时,实验组p H值明显高于气腹组,差异有统计学意义(P<0.01);24 h后两组差异无统计学意义(P>0.05)。30 min、60 min、90 min、24 h后,实验组TNF-α明显高于气腹组,差异有统计学意义(P<0.05);48 h、72 h时,两组差异无统计学意义(P>0.05)。结论:大鼠腹腔灌注5%碳酸氢钠溶液后可提升术中腹腔局部微环境p H值,保护了CO2气腹腹腔镜手术后腹膜巨噬细胞产生TNF-α的功能,可能会减少肿瘤腹腔内播散及穿刺孔转移的发生。
Objective:Through the method of intraperitoneal infusion of 5% Na HCO3 into abdominal cavity of rats,the study is to observe the influence of CO2 pneumoperitoneum on peritoneal fluid p H and TNF-α,to evaluate its effectiveness,and to explore its mechanism,in order to provide a theoretical basis for the clinical laparoscopic surgery in the field of malignancy treatment. Methods:Thirty healthy Wistar rats were selected and divided into three groups. Experimental group:10 ml of 5% Na HCO3 solution was applied to the rats for intraperitoneal lavage to establish the CO2 pneumoperitoneum. Pure CO2 pneumoperitoneum group:dynamic CO2 pneumoperitoneum maintained for 90 min. Each group drained liquid after 90 min. The control group was only performed simple anesthesia without pneumoperitoneum. After 30 min,10 ml of 0. 9% Na Cl solution was applied to the rats of experimental group for intraperitoneal lavage and 20 ml of 0. 9% Na Cl solution was given to the pure CO2 pneumoperitoneum group and the control group respectively. After30 min,60 min,90 min,24 h,48 h and 72 h,peritoneal fluid samples were taken to observe the p H value of the peritoneal fluid. The enzyme-linked immunosorbent assay method was used for the measurement of the TNF-α. Results:p H value:after 30 min,60 min and90 min,the value of the experimental group was significantly higher than that in the CO2 pneumoperitoneum group,and the difference was statistically significant( P〈0. 01);there was no significant difference after 24 h( P〉0. 05). TNF-α:after 30 min,60 min,90 min and 24 h,for the experimental group and CO2 pneumoperitoneum group,the TNF-α produced from the experimental group was significantly higher than that from CO2 pneumoperitoneum group and the difference was statistically significant( P〈0. 05);After 48 h and72 h,there was no significant difference between two groups( P〉0. 05).
