摘要 目的观察开腹手术和腹腔镜手术时不同腹腔环境对大鼠血液和腹腔pH值的影响。方法将80只雌性SD大鼠随机分为4组:开腹组、CO2气腹组、无气组、N2气腹组,每组20只。分别模拟开腹和不同腹腔镜腹腔环境,时间为1h。手术前后检测血液和腹腔pH值,评价不同方法对全身和局部酸碱环境的影响。结果术前各组间血液和腹腔pH值无差异(P>0.05);术毕CO2气腹组血液和腹腔pH值较术前明显降低(P<0.01),并低于其他各组(P<0.01);N2组、开腹组和无气组血液和腹腔pH值无明显变化(P>0.05)。结论CO2气腹导致血液和腹腔pH值明显降低,对腹腔pH值影响尤为显著,导致腹腔局部酸碱平衡紊乱。 [Obiective] To observe the effect of laparotomy and various laparoscopic coeliac environment on pH value of the arterial blood and abdominal cavity in a rat model. [Methods] Eighty female S-D mrs were divided randomly into four groups: laparotomy, carbon dioxide pneumoperitoneum, gasless and nitrogen gas pneumoperitoneum, twenty rats of each group. Simulate the laparotomy and various laparoscopic pneumoperitoneum procedure for lh every sample. Pre and postoperative carotid artery blood gas and abdominal cavity pH were detected to evaluate the changes of the systemic and local acid-base balance. [Results] There were no differences of blood and abdominal cavity pH value among every groups before intervention (P 〉0.05). However there were significant decrease of pH value both in blood and abdominal cavity in CO2, group after surgery (P 〈0.01) when compared with other groups. There were no significant changes of pH value among the other three groups (P 〉0.05). [Conclusions] There were significant decrease of pH value both in blood and abdominal cavity in CO2 pneumoperitoneumon group, especially the changes in abdominal cavity, which were important to the local acid-base imbalance.
