SSA、SSB及SS56在干燥综合征患者唇腺组织中表达的研究 被引量:8

摘要 目的:检测干燥综合征(Sjogren syndrome,SS)患者唇腺组织中的SSA、SSB及SS56蛋白的表达,分析比较三者在唇腺中表达的阳性率,从而探讨SS56在SS发病机制及诊断中的意义。方法:采用免疫组化法测定30例原发性干燥综合征(primary Sjogren syndrome,p SS)患者、11例继发性干燥综合征(secondary Sjogren syndrome,s SS)患者及5例正常人唇腺组织中SSA60、SSB及SS56蛋白的表达,分析41例SS患者的临床表现及实验室检查结果。结果:1SSA、SSB及SS56在SS患者唇腺导管上皮细胞(DEC)和腺泡上皮细胞(AEC)中都有阳性表达,后二者在淋巴细胞中的表达高于前者;23种蛋白在p SS和s SS两组中的表达差异无统计学意义(P>0.05);3种蛋白在SS患者与正常对照组的表达差异有统计学意义(P<0.05);3SSA与SSB阳性表达和SSA与SS56阳性表达比较,差异均有统计学意义(P<0.05),SSB与SS56间表达差异无统计学意义(P>0.05);4SS56阳性组与阴性组的临床表现比较,差异无统计学意义(P>0.05);实验室检查中红细胞、血红蛋白及血沉,两组比较差异有统计学意义(P<0.05)。结论:SSA、SSB及SS56在SS患者唇腺组织中均有阳性表达,且后二者的阳性表达高于前者;SSA、SSB两种抗体的表达可能有组织特异性;SS56有望成为SS的新的诊断指标,SS56可能为疾病活动的指标之一。 Objective: To assess the expression levels of SSA, SSB and SS56 protein in the labial salivary glands of pa- tients with Sjogren's syndrome(SS), in order to explore the significance of SS56 protein in the pathogenesis and diagnosis of SS. Method: Immunohistochemical staining technique was used to detect the expression of SSA60, SSB and SS56 protein in the labial gland from 30 cases of primary SS, 11 cases of secondary SS(sSS), and 5 normal controls. The clinical manifesta- tions and laboratory tests of 41 SS patients were analyzed. Results: ① Both lip gland ductal epithelial cells(DEC) and aci- nar epithelial cells(AEC) were positive for SSA, SSB and SS56 in SS patients. The expression of SSB and SS56 was stronger than that of SSA in the lymphocytes. ②The expression of all three proteins was not different between pSS and sSS group (P〉0.05), while the differences between SS patients and normal controls were significant (P〈0.05). ③ The differences be- tween SSA and both SSB and SS56 were also significant (P〈0.05). There were no significant differences in the expression between SSB and SS56(P〉0.05). ④The clinical manifestations between SS56 positive and negative group were no differences (P〉0.05). In contrast, the differences of the red blood cells, hemoglobin and erythrocyte sedimentation rate were significant between these two groups(P〈0.05). Conclusions: The expression of SSA, SSB and SS56 was all positive in the labial glands of SS patients. The positive rates of SSB and SS56 expression were higher than that of SSA. The expression of SSA and SSB is tissue specific. SS56 could be a new indicator for both the diagnosis and the status of SS.
