摘要
目的评价超声引导下半自动活检对浅表淋巴结转移癌的诊断效果。方法在超声引导下采用半自动切割活检针对59例浅表肿大淋巴结徒手穿刺活检。将穿刺组织的病理结果与术后病理或临床随访结果进行对照,评价该技术对浅表淋巴结转移癌诊断效果。结果 59例病例中共对59个肿大淋巴结进行穿刺活检,病例取材成功率为100%(59/59),穿刺取材成功率为80.14%(113/141),穿刺病理组织学诊断满意率为98.31%(58/59)。穿刺病理组织学结果:淋巴结转移性癌35例(阳性),非淋巴结转移癌24例(阴性)。术后病理或临床随访结果:淋巴结转移癌36例(A组),非淋巴结转移癌23例(B组)。该技术对浅表淋巴结转移癌的诊断敏感度为97.22%(35/36),特异度为95.83%(23/24),准确率为98.31%(58/59)。结论超声引导下半自动活检对浅表淋巴结转移癌诊断具有敏感度高、特异度高、准确率高的特点。该技术安全,对恶性肿瘤的诊断具有重要意义。
Objective To evaluate the diagnostic effects of ultrasound-guided semi-automatic biopsy of superficial lymph node metastatic carcinoma. Methods Superficial lymphadenopathy of 59 patients were punctured by using semi-automatic biopsy nee-dle under ultrasound-guided. We compared the pathologic diagnosis of needle biopsy with operative pathologic diagnosis or clini-cal follow-up diagnosis,and evalute the diagnosis efficacy of this technique about superficial lymph node metastatic carcinoma. Results Fifty-nine lymph node of 59 patients were punctured for biopsy. The achievement ratio of cases sampling and puncture sampling was 100%(59/59) and 80.14%(113/141),respectively. The satisfaction of puncture histological pathology diagnose was 98. 31%(58/59). The diagnose results of puncture histological pathology:35 cases of lymph node metastatic carcinoma(Positive) ,24 cas-es of lymph node non-metastatic carcinoma (Negative). The pathology diagose results after operation or follow-up:36 cases of lymph node metastatic carcinoma (A group),23 cases of lymph node non-metastatic carcinoma (B group). The sensitivity of diag-nose,specificity and accuracy rating was 97.22%(35/36),95.83%(23/24) and 98.31%(58/59),respectively. Conclusion The ultra-sound-guided semi-automatic biopsy of superficial lymph node metastatic carcinoma has the characteristic of high sensitivity of diagnose,high specificity and high accuracy rating. This technique is safe and has an significant meaning of diagnosing cancer.
