摘要
目的探讨超声引导下平面内穿刺与传统盲穿PICC置管临床应用价值。方法纳入我院2016年5月-2017年11月行PICC置管477例,病例分为三组:A组381例,B组82例,C组14例。A组为肘部静脉可直视或可触及弹性,行传统盲穿置管病例;B组为肘部静脉不可直视且不可触及弹性,行超声引导下平面内穿刺置管病例;C组为肘部静脉可直视或可触及弹性,经2次传统盲穿仍然置管不成功,随后转为超声引导下平面内穿刺置管病例。结果与A组相比B组及C组一次穿刺成功率及置管成功率均更高。A组一次穿刺成功率及置管成功率分别为86.09%(328/381)、84.25%(321/381);B组一次穿刺成功率及置管成功率分别为93.9%(77/82)、91.46%(75/82);C组一次穿刺成功率及置管成功率分别为92.86%(13/14)、92.86%(13/14)。结论超声引导下平面内进针PICC置管穿刺成功率及置管成功率高,为那些血管条件不佳又需PICC置管的肿瘤化疗患者提供比传统盲穿更优越的PICC置管方法。
Objective To investigate the clinical value of peripherally inserted central catheter(PICC)using ultrasound-guided in-plane and traditional blind procedure.Methods 477 cases of PICC catheterization in our hospital from May 2016 to November 2017 were included,divided into three groups:group A(381 cases),group B(82 cases)and group C(14 cases).In group A,the elbow vein could be seen directly or touched,the traditional blind procedure was performed;In group B,the elbow vein could not be seen directly and touch,the ultrasound-guided in-plane was performed;In group C,the elbow vein could be seen directly or touch,the traditional blind procedure was performed,but the catheterization was still unsuccessful after two times of traditional blind procedure,and then the ultrasound-guided in-plane was performed.Results Compared with group A,the success rate of frist time procedure and frist time catheterization in group B and group C were higher.The success rate of frist time procedure and frist time catheterization in group A respectively were 86.09%(328/381),84.25%(321/381);those in group B respectively were 93.9%(77/82),91.46%(75/82);those in group C respectively were 92.86%(13/14),92.86%(13/14).Conclusion Ultrasound-guided in-plane has high catheterization success.Provide a better PICC method than the traditional blind procedure for those tumor chemotherapy patients with poor vascular conditions and need PICC.
