摘要
目的 分析并评价高血压患者颈动脉硬化与基质金属蛋白酶-3(MMP-3)基因型的相关性.方法 将本院自2010年5月至2013年5月期间收治的原发性高血压患者108例作为观察组,将102名健康体检者作为对照组.MMP-3基因启动子在-1171位点存在三类基因型,分别为6A/6A、5A/6A及5A/5A,对比三类基因型在两组中的分布情况.观察组高血压患者69例与对照组体检者31名采取颈动脉超声检查,对比分析两组的内-中膜厚度(IMT).结果 观察组中6A/6A基因型(65.7%)和6A等位基因频率(82.4%)高于对照组基因型(53.8%)和等位基因频率(75.9%),但分布差异无统计学意义(基因型P=0.092,等位基因P=-0.118);观察组6A/6A基因型患者在起始部位、膨大前及膨大部位的IMT均显著高于对照组(P<0.05),差异有统计学意义.患者年龄与舒张压是颈总动脉内径的关键影响因素;年龄、舒张压、体重指数是IMT的关键影响因素.结论 原发性高血压MMP-3基因型患者的颈动脉内-中膜厚度受多种危险因素影响,高血压6A/6A基因型患者其颈动脉内-中膜厚度增加.
Objective To analyze and evaluate the correlation of hypertension and carotid atherosclerosis and matrix metalloproteinase-3 (MMP-3) genotype in hypertension patients.Methods During 2010 May to 2013 May,108 cases of primary hypertension patients were choosen as the observation group,102 healthy persons as control group.The MMP-3 gene promoter has three kinds of genotypes including 6A/6A,5A/6A and 5A/5A in the-1171 locus,the distribution of those three kinds of genotypes in two groups was compared.The carotid ultrasonography was evaluated in 69 cases of observation group and 31 cases of control group,and the intima-media thickness(IMT) was compared between two groups.Results In the observation group,genotype 6A/6A (65.7%) and the frequency of 6A allele (82.4%) were higher than that of the control group (53.8%) genotype and allele frequency (75.9%),but the distribution difference was not statistically significant (compared with genotype P=0.092,allele P=0.118).The observation group patients with the 6A/6A genotype in the starting position,and expanded rapidly before parts of IMT were significantly higher than those in the control group (P〈0.05).The age of patients and levels of diastolic blood pressure were the key influence factors of carotid artery diameter,and the patients age,diastolic blood pressure,body weight index were the key factors of IMT.Conclusion In patients with primary hypertension carotid intima-media thickness is affected by many risk factors,the carotid artery 6A/6A genotype in patients with hypertensive disease of intima-media thickness increase.
