摘要
目的系统评价黄芪注射液对慢性充血性心力衰竭血流动力学的影响,评估其有效性及安全性。方法计算机检索PubMed、EMbase、the Cochrane Library等英文数据库及CNKI、Wanfang database、VIP、CBM等中文数据库,搜索所有关于黄芪注射液治疗慢性心衰的随机对照试验(randomized controlled trial, RCT),并追索纳入研究的参考文献,采用RevMan5.3软件进行有关数据的Meta分析。结果 Meta分析结果显示,黄芪注射液联合西药常规治疗提高CHF治疗的总有效率[RR=1.21,95%CI(1.16,1.27),P <0.000 01],增加射血分数[MD=5.93,95%CI(4.47,7.39),P <0.000 01]及每搏输出量[MD=5.23,95%CI(3.67,6.78),P <0.000 01],缩短左室舒张末径[MD=4.04,95%CI(1.11,6.97),P=0.007],改善6 min步行试验[MD=47.61,95%CI(14.09,81.12),P=0.005]。结论现有证据支持黄芪注射液联合西药常规治疗慢性心衰优于单纯西药治疗,仍需开展多中心、大样本、高质量的RCT进行验证。
Objective To systematically assess the hemodynamic effects of Huangqi injection in the treatment of chronic heart failure(CHF), and provide evidence for clinical reference. Methods We searched PubMed, EMbase, the Cochrane Library, CNKI, Wanfang database, VIP and CBM to collect randomized controlled trials(RCTs) and cross-over studies about Huangqi injection in the treatment of CHF and Meta-analysis is performed by RevMan5.3 software. Results The results of Meta-analysis show that, as compared with western medicine alone, Huangqi injection base on the western medicine can improve the total effective rate of the treatment of chronic heart failure,[RR = 1.21, 95 % CI( 1.16,1.27), P < 0.000 01], with increasing ejection fraction,[MD = 5.93, 95 % CI( 4.47, 7.39), P < 0.000 01], increased stroke volume,[MD = 5.23, 95 %CI(3.67, 6.78), P < 0.000 01], Shorten the left ventricular end-diastolic diameter, [MD = 4.04, 95 % CI(1.11, 6.97), P = 0.007], and improve the 6 min walk test, [MD = 47.61, 95 % CI(14.09, 81.12), P = 0.005]. Conclusion Current evidences support that applying Huangqi injection based on western medicine for chronic heart failure is better than routine treatment. Nevertheless, more RCTs with multi-center,a large sample size and high quality sample are sill required to verify the efficacy.
