摘要
目的探究大剂量黄芪注射液联合伊伐布雷定治疗难治性心力衰竭的临床疗效及对患者心肌纤维化的影响。方法选取我院2019年1~12月收治难治性心力衰竭患者94例,随机分为观察组和对照组,各47例。对照组给予常规治疗加用伊伐布雷定,观察组予以常规治疗+伊伐布雷定+大剂量黄芪注射液。对比两种治疗方法的疗效和安全性。结果治疗前两组患者氨基末端脑钠尿肽前体(NT-proBNP)、心肌肌钙蛋白-I(cTn-I)、左室射血分数(LVEF)、左心室舒张末期容积(LVEDV)、左心室收缩末期容积(LVESV)、基质金属蛋白酶-9(MMP-9)、转化生长因子-β(TGF-β)、结缔组织生长因子(CTGF)比较,差异均无统计学意义(P>0.05)。治疗后两组患者NT-proBNP、cTn-I、LVEDV、LVESV、MMP-9、TGF-β、CTGF等均显著降低,且观察组低于对照组;LVEF显著升高,且观察组高于对照组,差异均有统计学意义(P<0.05)。两组患者均未见明显药物相关不良反应。结论大剂量黄芪注射液联合伊伐布雷定对难治性心力衰竭患者有较好疗效和较高的安全性,并能有效阻止心肌纤维化。
Objective To study the effect of high dose astragalus injection combined with ivabradine in the treatment of refractory heart failure and its effect on myocardial fibrosis.Methods A total of 94 patients with refractory heart failure from January to December in 2019 were selected,47 patients in each follow-up group.The control group was given ivabradine,and the observation group was given ivabradine plus large dose of Astragalus injection.The efficacy and safety of the two treatment methods were compared.Results Before treatment,there was no statistical significance in NT-proBNP,cTn-I,LVEF,LVEDV,LVESV,MMP-9,TGF-βand CTGF between the two groups(P>0.05).After treatment,NT-proBNP,cTn-I,LVEDV,LVESV,MMP-9,TGF-β1 and CTGF in the two groups were significantly decreased,and the observation group was lower than the control group.LVEF in observation group was significantly higher than that in control group(P<0.05).There were no obvious drug-related adverse reactions in two groups.Conclusion High dose astragalus injection combined with ivabradine has good efficacy and safety in patients with refractory heart failure,and can effectively prevent myocardial fibrosis.
