摘要
目的观察糖尿病白内障超声乳化人工晶体植入术后泪膜的变化。方法选取58例糖尿病白内障患者为研究对象,对其行白内障超声乳化人工晶体植入术治疗,观察患者术后干眼症发生情况,比较患者术后1周、1个月、3个月干眼症状积分术前及术后1周、1个月、3个月SchirmerⅠ试验、泪膜破裂时间(BUT)检查及角膜荧光素染色(CFS)评分。结果术后有2例患者患有干眼症,干眼症发生率为3.4%。患者术后1周干眼症状积分为(1.81±0.52)分,术后1个月干眼症状积分为(1.24±0.32)分,术后3个月干眼症状积分为(0.65±0.12)分;患者术后3个月干眼症状积分均低于术后1周和术后1个月,差异有统计学意义(P<0.05)。术后1周、1个月SchirmerⅠ试验、泪膜破裂时间及角膜荧光素染色评分与术前比较差异有统计学意义(P<0.05);术后3个月SchirmerⅠ试验、泪膜破裂时间及角膜荧光素染色评分与术前比较,差异无统计学意义(P>0.05)。结论糖尿病白内障超声乳化人工晶体植入术患者术后1个月内泪膜稳定性较高,随着术后时间的推移泪膜稳定性逐渐降低,至术后3个月泪膜功能与术前差距不明显。
Objective To observe the changes of tear film after phacoemulsification and intraocular lens implantation in diabetic cataract. Methods A total of 58 diabetic cataract patients were selected as study subjects and treated with phacoemulsification and intraocular lens implantation. The occurrence of dry eye after operation was observed, and the scores of dry eye symptoms at 1 week, 1 month and 3 months after operation, Schirmer Ⅰ test, tear film break-up time(BUT) and corneal fluorescein staining(CFS) before operation, 1 week, 1 month and 3 months after operation were compared. Results There were 2 patients with dry eye after operation, and the incidence of dry eye was 3.4%. The score of dry eye symptoms was(1.81±0.52) points at 1 week after operation,(1.24±0.32) points at 1 month after operation, and(0.65±0.12) points at 3 months after operation. The scores of dry eye symptoms at 3 weeks after operation were lower than those of 1 week and 1 month after operation, and their difference was statistically significant(P<0.05). The Schirmer Ⅰ test, tear film break-up time and corneal fluorescein staining scores at 1 week and 1 month after operation were significantly different from those before operation(P<0.05). There was no statistically significant difference in Schirmer Ⅰ test, tear film break-up time and corneal fluorescein staining scores at 3 months after operation and before operation(P>0.05). Conclusion The tear film stability of patients with diabetic cataract after phacoemulsification and intraocular lens implantation is higher within 1 month, and gradually decreases with the passage of time after operation. The tear film function at 3 months after operation is not significantly different from that before operation.
