Effect of Femtosecond Laser-Assisted Phacoemulsification and Intraocular Lens Implantation on Astigmatism,Intraocular Pressure and Tear Film Functions in Cataract Patients with Corneal Astigmatism

摘要 目的探讨飞秒激光辅助超声乳化人工晶体植入术对白内障合并角膜散光患者散光程度、眼压及泪膜功能的影响。方法选取2018年9月至2019年12月,白内障合并角膜散光≥0.75 D的患者56例68只眼,根据手术方法不同分成两组,分别为对照组患者29例,应用传统白内障超声乳化人工晶体植入术治疗;观察组患者27例,应用飞秒激光辅助超声乳化人工晶体植入术治疗。比较两组患者术后1周、3个月散光程度、眼压及泪膜功能恢复情况。结果术后1周、3个月观察组角膜散光度、眼压均低于对照组(P<0.05)。术后1周、3个月观察组泪膜破裂时间(breakup time,BUT)高于对照组,基础泪液分泌试验(Schirmer I test(SIt),ST-Ⅱ)值测定低于对照组(P<0.01)。术后1周、3个月观察组裸眼远视力(uncorrected distant visual acuity,UDVA)、最佳矫正远视力(corrected distant visual acuity,CDVA)值均低于对照组(P<0.05)。结论飞秒激光辅助超声乳化人工晶体植入术可改善白内障合并角膜散光患者的散光程度,降低眼压,改善泪膜功能。 Objective To investigate the effect of femtosecond laser-assisted phacoemulsification and intraocular lens implantation on astigmatism,intraocular pressure and tear film function in cataract patients with corneal astigmatism.Methods Totally 56 patients of cataract who were complicated with corneal astigmatism(≥0.75 D)from Sep.2018 to Dec.2019 were selected and divided into two groups by surgical methods,namely,the control group and observation group.The 29 patients in the control group were treated with traditional phacoemulsification and intraocular lens implantation,and the 27 patients in the observation group with femtosecond laser assisted phacoemulsification and intraocular lens implantation.The astigmatism degree,intraocular pressure and tear film function recovery were compared between the two groups 1 week and 3 months after the operation respectively.Results The corneal astigmatism and intraocular pressure in the observation group were lower than those in the control group(P<0.05).The BUT in the observation group was higher than that in the control group and ST-Ⅱlower than that in the control group(P<0.01).The values of UDVA and CDVA in the observation group were lower than those in the control group(P<0.05).Conclusions Femtosecond laser-assisted phacoemulsification and intraocular lens implantation will improve the astigmatism of cataract patients with corneal astigmatism,reduce intraocular pressure and improve tear film functions.
