摘要
【目的】探讨飞秒激光在白内障超声乳化手术中的应用效果。【方法】选择2014年1月至2015年12月于本院进行白内障超声乳化手术的患者282例(297眼),根据是否辅以飞秒激光分为观察组(157例161眼)与对照组(125例136眼),观察组采用飞秒激光辅助超声乳化术,对照组仅采用传统超声乳化术,观察并记录手术过程中的超声能量(AVE)、有效超声时间(EPT),术后两个月术源性散光(SIA)、检查最佳矫正视力(BCVA)、角膜内皮细胞丢失率以及并发症。【结果】观察组手术过程中平均AVE为(6.5±2.1)%显著少于对照组(10.0±1.5)%,EPT为(6.1±2.0)S显著短于对照组(8.2±1.5)S,差异具有统计学意义(P〈0.01);观察组术后两个月SIA显著少于对照组,差异具有统计学意义(P〈0.05);两组患者术后BCVA之间的差异无统计学意义(P〉0.05);观察组术后角膜内皮细胞丢失率(8.4±4.3)%显著低于对照组(15.9±7.4)%,差异具有统计学意义(P〈0.05);两组均未发生严重并发症。【结论】飞秒激光在白内障超声乳化手术中具有良好的有效性与安全性,能够明显降低AVE与EPT,降低SIA和角膜内皮的丢失率,值得临床推广应用。
[Objective]To investigate the effect of femtosecond laser in cataract ultrasonic emulsification operation. [Methods]Two hundred and eighty-two cases (297 eyes) of patients underwent cataract ultrasonic emulsification operation in our hospital from January 2014 to December 2015 were divided according to the different operation mode into femtosecond laser assisted group (the observation group, 157 cases with 161 eyes) and the traditional ultrasonic phacoemulsification group (125 cases of 136 eyes).The observation group was treated with ultrasonic phacoemulsification assisted with femtosecond laser , while the control group with traditional ultrasonic phacoemulsification therapy .The process of the operation of the average ultrasonic energy (AVE), the effective phaco time(EPT), the surgical induced astigmatism 2 months postoperation(SIA), the best corrected visual acuity(BCVA), corneal endothelial cells lost rate and complications were observed and recorded. [Results] The average ultrasonic energy (AVE) in the process of operation of the femtosecond laser group was 6.5±2.1% ,which was significantly less than that of the conventional phacoemulsification group 10.0 ±1.5 %, the EPT was (6.1±2)s , which was significantly shorter than that of the control group (8.2 ± 1.5)s, the difference was statistically significant ( P 〈0.01). In the observation group, the SIA two months after operation was significantly less than that of the control group, the difference was statistically significant ( P 〈0.05). There was no significant difference in BCVA between the two groups ( P 〉0.05) ; The corneal endothelial cell loss rate in the observation group was (8.4 ±4.3)%, which was significantly lower than that of the control group (15.9 ±7.4)%, the difference was statistically significant ( P 〈0.05); no serious complications occurred in the two groups.[Conclusion] Femtosecond laser assistance is an effective and safe approach in cataract ultrasonic emulsific
