摘要
目的 评价 Bac T/Alert连续监测血培养系统检出儿科常见血培养分离菌的孵育时间。方法 分析 Bac T/Alert全自动血培养系统检出 1 42 6份新生儿和小儿阳性血培养瓶败血症病原菌的检出时间特点。结果 1 42 6份新生儿和小儿血培养标本被系统检出阳性培养瓶 1 3 3份 ,分离阳性率 9.3 3 % ,其中革兰阳性菌占 75 .8%、革兰阴性菌占 2 1 .9%、真菌占 2 .3 % ;1 3 3份阳性培养瓶被系统于 1 2 ,2 4 ,3 6 ,48,72 h及 96 h的检出率分别为 1 0 .4% ,6 7.5 % ,85 .4% ,93 .7% ,96 .7%和 99.7%。结论 Bac T/Aler T全自动血培养系统可在 48h以内检出 93 .7%的儿科败血症重要病原菌 ,实验室可在系统监测 2 d后发出血培养的一级报告 ,48h系统监测结果为阴性的培养瓶可能培养出真菌、四联球菌、溶血巴斯德菌及某些认为是污染的革兰阳性杆菌 ,可将系统监测时间确定为 5
Aim To evaluate the time of incubation to detect positive blood cultures from infants and children with suspected sepsis using a computer assisted,automated blood culture system(BacT/Alert).Method The time to positive blood cultures growing pediatric pathogens were analyzed.Results One hundred thirty three (9 33%)positive blood culture results were obtained from 1 426 infants and children Gram positive organisms accounted for 75 8% of the positive culture results,Gram negative organisms accounted for 21 9%,and yeast for 2 3%,Of the 133 positive blood cultures,10 4%were positive by 12 hours incubation,67 5% by 24 hours,85 4%by 36 hours,93 7%by 48 hours,96 7%by 72 hours and 99 7%by 96 hours.Conclusion Because 93 7%of all cultures containing pediatric pathogens were detected within the 48 hours incubation,the first report for the blood culture should be sent out after the bottle was loaded into the continuously monitoring blood culture system for 48 hours. The yeast,Micrococcus species,Pasteurella haemolytica and other nonpathogenic Gram positive rods might be isolated from the bottles which remained undetected by the BacT/Alert system for 48 hours, so the negative culture bottles were determined by 5 days incubation.
