摘要
目的 :通过分析不同病变组织表达癌胚抗原mRNA(CEA mRNA)的差异 ,探讨CEA mRNA指标诊断消化道恶性肿瘤的应用价值。方法 :以 35例消化道恶性肿瘤病人和 12例非恶性肿瘤的良性消化道病人为研究对象 ,收集新鲜肿瘤组织及其癌旁正常组织和非恶性肿瘤组织以及同体静脉血液为检测标本 ,应用套式反转录聚合酶链反应 (RT PCR)检测组织标本CEA mRNA、酶联免疫吸附试验 (ELISA)检测血液标本癌胚抗原 (CEA)。结果 :恶性消化道肿瘤病人肿瘤组织标本 2 9例 (82 86 % )表达CEA mRNA、血液标本 15例 (4 2 86 % )表达CEA ,两个指标表达情况的差别具有显著性 (P <0 0 1)。癌旁正常组织和非癌病理组织也分别检测出 4例和 1例表达CEA mRNA阳性结果 ,组织标本CEA mRNA指标诊断恶性消化道肿瘤的特异性为 91 6 7%、敏感性为 82 86 %。结论 :在诊断消化道恶性肿瘤方面 ,CEA mRNA是一个特异性和敏感性较好且很实用的指标 ,可以常规应用。
Purpose:To evaluate the differences in expression of carcinoembryonic antigen mRNA (CEA mRNA) in various tissues and its use in the diagnosis of digestive tract carcinoma.Methods:There were 35 patients with digestive tract carcinoma and 12 patients with non carcinoma disease in the study. The expression of CEA mRNA in the digestive tract tissues specimens was detected with nested reverse transcriptase polymerase chain reaction(RT PCR). A contrast test was detection of carcinoembryonic antigen (CEA) in blood sepcimen with enzyme linked immunosorbent assay (ELISA). Results:The expression of CEA mRNA in cancer tissues with RT PCR was 29 (82.86%) with positive results and that of CEA in blood with ELISA was 15 (42.86%) in the patients with digestive tract carcinoma. The difference in positive results was significant ( P <0.01) between CEA mRNA and CEA. There were 4 positive results of CEA mRNA in 35 normal tissues around the carcinoma tissue and 1 positive result of CEA mRNA in 12 non carcinoma tissues with RT PCR in the contrast specimens the non carcinoma tissues and the normal tissues around the carcinoma tissues. The specificity was 91.67% and the sensitivity was 82.86% on the expression of CEA mRNA for detection of digestive tract carcinoma.Conclusions:CEA mRNA is a useful and better index for laboratory diagnosis of digestive tract carcinoma. The index of CEA mRNA with RT PCR is suitable for routine use. [
