HPV E6蛋白与宫颈HPV持续感染及宫颈癌发病的相关性研究 被引量:18

摘要 目的:探索HPV16/18 E6蛋白与宫颈人乳头瘤病毒持续感染的关系。方法:选择HPV16/18 E6蛋白的ELISA技术与等离子表面振谐(SPR)进行HPV基因分型,对389例宫颈病变的患者进行了检测。结果:共检出HPV感染120例,HPV总阳性率为30.85%。并且HPV阳性率随着宫颈病变的逐渐加重呈趋势性增高。其中慢性宫颈炎的HPV阳性率为12.05%、宫颈上皮内瘤变的HPV阳性率为为53.55%、宫颈癌的HPV阳性率为100.00%。3组的HPV感染率差异具有统计学意义(P<0.05)。感染率最高的亚型是HPV16/18型,感染率为29.31%,占HPV各型感染的79.17%,且与宫颈病变严重程度呈正相关,其在宫颈上皮内瘤变和宫颈癌的感染率达67.95%,明显高于其他组(P<0.05)。389例标本中,共检出HPV16/18 E6蛋白96例,HPV E6蛋白总阳性率为24.68%。HPV E6蛋白阳性率随着宫颈病变的逐渐加重呈趋势性增高。慢性宫颈炎组E6蛋白阳性率为2.68%,宫颈上皮内瘤变组E6蛋白阳性率为52.26%,宫颈癌组E6蛋白阳性率为100.00%。3组的HPVE6蛋白阳性率差异具有统计学意义(P<0.05)。27例HPV DNA阳性的慢性宫颈炎患者中,在6个月后有6例HPV DNA转阴,HPV DNA中的基因型均为低危型(HPV11型2例、HPV42型2例、HPV44型2例)。25例无明显宫颈液基细胞学改变,而6例HPV16/18 E6蛋白阳性的慢性宫颈炎患者中,无HPV DNA转阴,在6个月后有2例宫颈液基细胞学改变,经活检确诊为宫颈上皮内瘤变CINⅠ级,并且HPV16/18 E6蛋白持续阳性。在19例HPV DNA阳性的宫颈上皮内瘤变CINⅠ患者,在6个月后有1例HPV DNA转阴(HPV DNA中的基因型为11型),16例无明显宫颈液基细胞学改变,而17例HPV16/18 E6蛋白阳性的宫颈上皮内瘤变CINⅠ级患者中,在6个月后,无HPV DNA转阴,有3例宫颈液基细胞学改变,经活检确诊为宫颈上皮内瘤变CINⅡ级,其HPV16/18 E6蛋白也呈现持续阳性。HPV16/18 E6蛋白阳性组HPV持续感染率在宫颈上皮内瘤变发生与发展中明显高于HPV1
