摘要
目的初步探讨骨形态发生蛋白2(BMP-2)和血管内皮生长因子(VEGF)单独及联合应用修复海水浸泡开放性骨缺损的疗效。方法将60只新西兰兔双侧桡骨中段制作1.5 cm骨缺损,海水浸泡3 h,随机分为A、B、C、D四组,每组15只兔。A组为对照组,植入自体髂骨;B、C、D三组为实验组,B组植入自体髂骨/可吸收胶原海绵(ACS)/BMP-2;C组植入自体髂骨/ACS/VEGF;D组植入自体髂骨/ACS/BMP-2/VEGF。分别于术后1、2、4、8及12周切取标本,每组每个时相点根据随机原则取6个标本,通过X线摄片、骨痂灰度值、组织形态学观察、免疫组织化学及环境扫描电镜观察各时相点骨缺损修复情况。结果①1、2周时,四组X线差异不明显,4、8、12周时B、C、D三组与A组的骨痂灰度值比较,差异有统计学意义(P<0.05);D组与B、C两组平均骨痂灰度值比较差异有统计学意义(P<0.05);B、C、两组平均骨痂灰度值比较差异无统计学意义(P>0.05)。②免疫组织化学结果表明:骨缺损修复早期BMP-2、VEGF二者相互促进,联合应用具有协同效应。结论①BMP-2、VEGF单独应用均能促进海水浸泡兔桡骨开放性骨缺损的修复。②BMP-2和VEGF联合应用能显著促进海水浸泡开放性骨缺损修复,两者联合应用具有协同效应。
Objective To investigate the efficacy of BMP-2 and VEGF single and combined repair application for open bone defect with seawater immersion.Methods Sixty New Zealand rabbits were divided into A,B,C,and D groups randomly and averagely.1.5cm bone defect was made using the rabbits of bilateral radius and then they were put into the seawater for three hours.Group A was as control group,into which autologous iliac was implanted;Group B,C and D was as experimental group;iliac,absorbable collagen sponge(ACS) and BMP-2 were implanted into group B,iliac,ACS and VEGF into group C,and iliac,ACS,BMP-2 and VEGF into group D.Animals were sacrificed and cut specimens in the 1st,2nd,4th,8th,12th week after grafting.Each group took six specimens for each phase randomly and the healing after grafting for the open defect under seawater immersion was observed by imaging observation,histologic observation,immunohistochemistry and environmental scanning electron microscopy.Results ①X-ray in every group rabbits had no significant difference during 1st and 2nd week.During 4th,8th and 12th week,the difference in callus gray value of groups B,C and D was statistically significant compared with that of group A(P&lt;0.05);the difference in callus gray value between group B,C and group D was statistically significant(P&lt;0.05);the callus gray value between group B and C showed no significant difference(P&gt;0.05).② The results of immunohistochemistry showed that BMP-2 and VEGF mutually promoted in early bone defect repairing,and the combined application of the two had a synergistic effect.Conclusion ①Both BMP-2 and VEGF can individually promote seawater immersion open bone defect repair.② The combined application of BMP-2 and VEGF can promote remarkably the seawater immersion open bone defect repair,and the combined application has synergetic effects.
