摘要
目的探讨一种新的切除丘脑肿瘤的方法并评价其治疗效果。方法回顾性分析27例丘脑肿瘤经开颅手术治疗的临床资料。结果全切14例,次全切10例,大部分切除3例。术后各种神经功能及体征较术前明显改善14例,无明显变化8例,病情加重4例,死亡1例。随访结果,其中可正常生活10例,基本生活能自理12例,严重功能障碍4例,死亡1例。术后并发脑积水11例,复发7例。结论丘脑肿瘤好发于低年龄段,以低级别胶质瘤多见;选择适当的手术方式,可以取得满意的治疗效果;辅以神经导航技术可以提高这类手术的成功率,降低术后并发症。
Objective To investigate a new operation way for the thalamic tumors and to evaluate therapeutic effect. Methods A re trospective analysis of clinical data of 27 cases with thalamic tumors by craniotomy in our hospital. Results In all 27 cases thalamic tumors, 14 received total resection, 10 subtotal and 3 greatly partial. 14 cases of postoperation the neurological function and symptoms were improved com- pared with the preoperative. There were 8 cases had no significant change,4 cases exacerbations and one cases died. After following -up,the cases which can live a normal life in 10 cases ,the basic live independently in 12 cases ,serious dysfunction in 4 cases and one case died. 11 ca- ses were developed hydrocephalus after operation,7 cases recurrenced. Conclusion The thalamic tumors predilection in low age,the low grade gliomas are in the majority. To select appropriate operation approach will get satisfactory therapeutic outcome. Especially with the neuronavigation＇s help the success ratio of the thalamic tumors operation improved,the operative complications to decrease.
