医院内泌尿系统感染病原菌分布及耐药性分析 被引量:18

摘要 目的探讨医院内引起泌尿系统感染常见病原菌及体外耐药现状,为临床合理使用抗菌药物提供依据。方法对医院2010年5月-2011年12月门诊及住院患者尿液标本中分离出的150株病原菌进行细菌鉴定及体外药敏试验。结果 150株泌尿系感染病原菌前5位以大肠埃希菌为主,占46.0%;其次金黄色葡萄球菌13株占8.7%、粪肠球菌9株占6.0%、凝固酶阴性葡萄球菌和奇异变形菌各8株,各占5.3%;泌尿系病原菌对常用抗菌药物产生了一定的耐药性,药敏显示,革兰阴性菌对亚胺培南、美罗培南100.0%敏感,对哌拉西林/他唑巴坦、头孢西丁、阿米卡星较敏感,敏感率均>90.0%,对氨苄西林敏感率<10.0%;革兰阳性菌对利奈唑胺、万古霉素敏感性最高,敏感率为100.0%,对莫西沙星敏感,敏感率>85.0%,对青霉素G敏感率<15.0%;产超广谱β-内酰胺酶(ESBLs)大肠埃希菌检出率为59.7%。结论病原菌耐药率呈上升趋势,对引起泌尿系统感染病原菌应及时、快速、准确地进行鉴定及药敏试验,为临床合理使用抗菌剂提供依据。 OBJECTIVE To observe the distribution and drug resistance characteristics of the pathogens causing hospital-acquired urinary system infections so as to provide basis for the reasonable use of antibiotics.METHODS Totally 150isolates collected from urinary infection specimens were identified and the drug susceptibility testing was performed from May 2010to Dec 2011.RESULTS Among 150strains of pathogens causing the urinary system infections,46.0% were Escherichia coli,followed by Staphylococcus aureus(13strains,8.7%),Enterococcus faecalis(9strains,6.0%),and coagulase-negative Staphylococci(8strains,5.3%)and Proteus mirabilis(8 cases,5.3%).The pathogens causing urinary tract infections produced certain drug resistance to the commonly used antibiotics,the drug susceptibility testing indicated that the gram-negative bacteria were 100.0%sensitive to imipenem and meropenem,the drug susceptibility rates to piperacillin-tazobactam,cefoxitin and amikacin were higher than 90.0%,and the drug susceptibility rate to ampicillin was below 10.0%;the gram-positive bacteria were most sensitive to linezolid and vancomycin,with the drug susceptibility rate of 100.0%,and the drug susceptibility rate to moxifloxacin was higher than 85.0%,the penicillin lower than 15.0%;the detection rate of extended spectrum beta-lactamases(ESBLs)-producing Escherichia coli was 59.7%.CONCLUSION The drug resistance rate of the pathogens keeps an upward trend.It is necessary to perform the timely,rapid,and accurate identification of the pathogens causing urinary system infections and to carry out the drug susceptibility testing so as to provide basis for the reasonable use of antimicrobial agents.
