地塞米松对碱烧伤后大鼠角膜核转录因子-κB活化的影响 被引量:9

摘要 目的探讨1g·L-1地塞米松对大鼠角膜碱烧伤后核转录因子-κB(nuclear factor kappa B,NF-κB)活化的影响。方法 24只SD大鼠右眼角膜碱烧伤后随机分为地塞米松组(造模后1g·L-1地塞米松滴眼)和生理盐水组(造模后生理盐水滴眼),每日滴眼4次,连续7d。碱烧伤后7d,裂隙灯观察角膜新生血管、炎症和角膜上皮缺损情况;制备角膜标本,采用苏木精-伊红染色法检测角膜组织病理学变化;免疫荧光染色检测角膜组织中NF-κB活化率,ELISA法测定IL-1β和VEGF蛋白表达水平。结果碱烧伤后7d,NF-κB活化率比较,角膜基质层地塞米松组为3.81%,生理盐水组为9.58%,差异有统计学意义(P<0.01);角膜上皮层地塞米松组为14.91%,生理盐水组为15.12%,差异无统计学意义(P=0.70)。角膜新生血管面积、炎症指数、IL-1β和VEGF表达水平,地塞米松组分别为(13.15±0.93)mm2、0.27±0.08、(105.71±14.77)ng·L-1和(532.86±74.79)ng·L-1,均低于生理盐水组的(30.07±4.32)mm2、0.65±0.21、(178.17±10.96)ng·L-1和(795.01±124.14)ng·L-1,差异均有统计学意义(均为P<0.01)。碱烧伤后7d,两组角膜上皮细胞均完整覆盖伤区。结论地塞米松抑制大鼠角膜碱烧伤后炎症和新生血管的作用与抑制角膜基质细胞内NF-κB的活化有关,地塞米松不影响角膜上皮的修复。碱烧伤后1周内,1g·L-1地塞米松眼液滴眼是安全、有效的治疗措施。 Objective To determine the effect of 1 g ·L-1 dexamethasone on NF-κB activation in cornea alkali burn rats. Methods A total of 24 SD rats with cornea alkali burn were randomly divided into dexamethasone group(DEX) and normal saline group(NS). Each group was treated with 1g·L-1 DEX or NS 4 times per day for the first 7 days. Cornea neovascularization (CNV) area, corneal inflammation index and epithelial wound healing were examined by slit-lamp microscope after cornea alkali burn for 7 days. Cornea samples were made and histopathological examination was performed by hematoxylin eosin staining;activation of NF-κB was measured by imnmnofluorescence assay and expression levels of IL-1β and VEGF were quantified by enzymelinked immunosorbant assay(ELISA). Results On the 7th day after cauterization, in the corneal stroma, the activation ratios of NF-κB were 3. 81% in DEX group and 9.58% in NS group respectively,so there was significant difference between them( P 〈 0. 01 ) ;in the epithelium,the activation ratios of NF-κB were 14.91% in DEX group and 15.12% in NS group respectively, so there was no significant difference between them (P = 0.70). The area of CNV, inflammation index, and the levels of IL-1β and VEGF in DEX group,which were ( 13.15 ±0.93)mm2 ,0.27 ±0.08, ( 105.71 ± 14.77)ng·L-1 and (532.86 ± 74.79 )ng ·L-1, respectively, were all significantly lower than those in NS group,which were (30.07 ±4.32)mm2,0. 65 ±0.21, (178. 17 ± 10.96)ng·L-1 and (795.01 ± 124. 14 )ng ·L-1, respectively ( all P 〈 0.01 ). Both group had full epithelial closure on the 7th day. Conclusion The inhibition effect of dexamethasone on corneal inflammation and neovascularization is related with the activation of NF-κB and dexamethasone dose not retard the corneal epithelial healing. Topical application of 1 g ·L-1 dexexamethasone is safe and effective at the 1 st week after cornea alkali burn.
