摘要
目的分析人类端粒酶反转录酶(hTERT)及基质金属蛋白酶-7(MMP-7)mRNA在贲门癌患者外周血中的表达。方法采用实时荧光定量反转录-聚合酶链反应(RT—PCR)法检测157例贲门癌患者术前血液标本中MMP.7mRNA和hTERTmRNA表达,对其表达与临床病理参数的关系进行比较。对无转移的81例患者术后6、12个月进行随访,根据患者临床症状、体征相应检查,判断复发转移。并将MMP-7mRNA和hTERTmRNA表达阳性组与阴性组复发转移情况进行比较。结果157例贲门癌患者中,hTERT阳性表达73例(46.0%);MMP.7阳性表达61例(38.8%)。hTERT、MMP-7表达与性别(X^2值为2.597、0.199)、年龄(X^2值为4.314、0.073)等因素无相关性(均P〉0.05),但与TNM分期(X^2值为10.624、14.530)、有无远处转移(X^2值为7.294、12.824)及组织的分化程度(X^2值为12.003、6.482)之间存在相关性(均P〈0.05)。术后随访6、12个月肿瘤转移率比较差异有统计学意义(P〈0.05)。血行微转移阳性患者术后12个月肿瘤转移概率是阴性患者的6.12倍(OR=6.120)。结论贲门癌患者外周血hTERT及MMP-7mRNA的异常表达与贲门癌的发展和转移有关,血液微转移检测对贲门癌早期发现及预后具有重要意义,值得进-步研究。
Objective To detect the expressions of human telomerase tranacriptase (hTERT) and MMP-7 mRNA in the peripheral blood of patients with cardia cancer. Methods Total RNA was extracted with TRIzol and was transcribed reversely into cDNA. RT-PCR was used to detect mRNA expression of MMP-7 and hTERT in the peripheral blood of 157 patients with cardia cancer. 81 patients without metastasis were followed up for 12 months. Results Of 157 patients, 73 cases (46.0 %) and 61 cases (38.8 %) had the positive expressions of hTERT, MMP-7, respectively. There were significant correlations between the blood micro-metastases MMP-7 and hTERT expressions and TNM staging (X2 = 10.624, 14.530), tumor metastasis (X2 = 7.294, 12.824), and differentiation degree (9(2 = 12.003, 6.482), but no significant correlations were seen in sex (X2 = 2.597, 0.199) and age (X2 = 4.314, 0.073). There was statistically significant in the occurrence rate of tumor metastasis between 6-month and 12-month follow-up. The occurrence rate of metastasis in patients with micro-metastases was 6.12-fold compared with patients without micro-metastases after 12-month follow-up. Conclusion Detection of MMP-7 and hTERT expressions can be used as reliable markers to predict peripheral blood micro-metastasis in patients with cardia carcinoma.
