肺表面活性物质鼻内给药对豚鼠急性分泌性中耳炎的治疗作用 被引量:2

摘要 目的研究肺表面活性物质鼻内给药对豚鼠急性分泌性中耳炎的治疗作用,为临床应用肺表面活性物质治疗分泌性中耳炎奠定基础。方法豚鼠鼓室内注射灭活的流感嗜血杆菌,建立豚鼠急性分泌性中耳炎动物模型(听力下降、中耳积液)。50只豚鼠随机取10只作为正常组(A组),40只中31只双耳造模成功,随机分为造模组(B组)10只;生理盐水组(C组)10只;肺表面活性物质组(PS组,D组)11只。观察各组鼓膜形态,记录声导抗鼓室图和听性脑干反应(ABR)阈值,计量资料采用单因素方差分析。结果 A组鼓膜透亮,光锥形态正常;鼓室图A型;ABR阈值为(14.1±4.8)dBHL。B组鼓膜浑浊,光锥消失,鼓室积液;鼓室图为B型或C型;ABR阈值提高至(51.2±6.3)dBHL,与A组比较差异有统计学意义。C组鼓室积液无明显变化,鼓膜浑浊;ABR阈值为(52.8±7.1)dBHL,与B组比较差异无统计学差异。肺表面活性物质滴鼻7d后,D组鼓室积液减少或消失;鼓室负压降低;ABR阈值降至(27.4±8.8)dBHL,与B组、C组比较差异有统计学意义。结论肺表面活性物质滴鼻给药可以显著降低分泌性中耳炎豚鼠的中耳负压和ABR阈值。 Objective To study the therapeutic effect of intranasal pulmonary surfactant (PS) drops on acute otitis media with effusion (OME) in guinea pigs. Methods Nonviable heat-killed Hemophilies influenzae solution was inoculated into the middle ear cavity of guinea pig via a transeardrum approach for the establishment of acute OME models in guinea pigs. Ten of 50 guinea pigs were taken randomly as normal group (A) ; 31 of the other 40 ones were successful- ly established the OME models (hearing loss and bilateral ears effusion) and then were randomly divided into 3 groups: 10 in model-control group (OME group, B), 10 in saline group (C) and 11 in PS group (D). Changes in morphology of tympanic membrane (TM), middle ear pressure and response threshold were recorded by otoscopy, tympanometry and auditory brainstem response (ABR) respectively. The data were analyzed with one-way ANOVA. Results Eardrum was clear in normal group (A), with type-A tympanogram and hearing threshold at ( 14. 1±4.8) dB HL. Serous effusion was present in the middle ear cavities of guinea pigs in OME group (B), with light cone disappearing, tympanogram turning from type-A to type-B or type-C, ABR threshold raising to (51.2±6.3 ) dB HL which was significantly different from that in normal group (A). The serous effusion of tympanum remained in saline group (C), with muddy eardrum, response threshold at (52.8±7.1 ) dB HL, with no statistical significance compared with OME group (B). After 7 days of intranasal dripping of pulmonary surfactant in PS group (D), serous effusion of tympanum reduced or disappeared, with negative middle ear pressure in tympanometry corrected, response threshold decreased to (27.4±8.8) dB HL with significant difference compared with OME (B) and saline (C) groups. Conclusions Pulmonary surfactant might play an important role in correcting negative pressure in middle ears of guinea pigs suffering from OME.
