摘要 目的 :探讨咽鼓管表面活性物质 (eustachiantubesurfactant,ETS)中相关蛋白的活性作用 .方法 :采用可控闭泡式表面张力测定仪进行豚鼠咽鼓管冲洗液原液、磷脂、相关蛋白的体外活性测定 ,由可读显微镜和计算机算出最大表面张力 (γmax)、最小表面张力 (γmin)及稳定系数 (S) ;通过低压舱上升实验建立气压损伤性中耳炎 (barotitismedia ,BM )动物模型 ,观察分别应用咽鼓管灌洗液、咽鼓管表面活性物质磷脂组分、磷脂 +蛋白、生理盐水冲洗的BM豚鼠咽鼓管被动开放压(pressureopeninglevel,POL)的变化 ,进行体内活性观察 .结果 :体外观察 :生理盐水组γmin和γmax均为最大 ,而S为最小 ;原液组γmin和γmax为最小 ,S大于生理盐水组 ,却低于蛋白组和磷脂组 ;磷脂组γmax和γmin介于原液与生理盐水之间 ,S与原液组接近 ;蛋白组γmax最大 ,γmin则仅略大于其他组 .体内观察 :咽鼓管灌洗液使BM的POL由 (12 .0± 1.9)kPa降低至 (6 .1± 1.7)kPa,磷脂 +蛋白组使POL由 (11.9± 1.9)kPa降低至 (6 .7± 1.7)kPa,均具有显著性差异 (P <0 .0 1) ,而ETS与磷脂 +蛋白组之间的活性作用无明显差异 ;对照的生理盐水使POL由 (11.9± 1.8)kPa变为 (11.7± 1.6 )kPa,无差异 ;而磷脂组分使POL由 (11.9± 1.6 )kPa降低至 (9.3± 1.9) AIM: To investigate the active effect of eustachian tube surfactant (ETS) associated protein. METHODS: The surface tension lowering properties in surfactant were measured by means of captive bubble surface tensiometer. The maximal, minimal surface tension ( γ max , γ min ) and stability constant ( S ) of original fluid (ETS), phospholipid + saline, protein + saline, were determined with saline as the control; Twenty barotitis media models with guinea pigs were successfully established by using the fight simulated test. The middle ear pressure necessary to force the eustachian tube to open (pressure opening level, POL) in barotitis media under flushing of middle ear with ETS, phospholipid, phospholipid + protein and saline were tested. RESULTS: In vitro : For normal saline (NS), γ max and γ min were the highest in these four samples, and S was the lowest. For original fluid, γ max and γ min were the lowest, and S was higher than that of NS but lower than those of phospholipid and protein. For phospholipid, γ max and γ min were in the middle between those of original fluid. For protein, γ max was near that of NS and γ min a little higher than that of original fluid, while S was the highest in these four; In vivo : phospholipid + protein reduced the POL from (11.9±1.9) kPa to (6.7±1.7) kPa; After flushing of ETS, the POL decreased from (12.0±1.9) kPa to (6.1±1.7) kPa. The differences between the ETS groups and the saline group were statistically significant, while there are not significantly difference between the ETS group and the phospholipid + protein group. In the control, saline lowered the POL from (11.9±1.8) kPa to (11.7± 1.6) kPa,there being no significantly difference. Phospholipid group significantly reduced the POL from (11.9±1.6) kPa to (9.3±1.9) kPa, but there is a relative reduction in the activity in comparison with those of the ETS group, and the phospholipid + pr
