摘要
目的探讨围生期窒息新生儿脐动脉血的pH、二氧化碳分压(PCO2)、氧分压(PO2)及剩余碱(BE)变化与喂养不耐受的关系。方法选择本院出生的新生儿189例,按Apgar评分标准分为两组:对照组93例,为大于8分的健康出生的新生儿;窒息组96例,为低于8分的新生儿;分别于出生后即做脐动脉血气分析。窒息组患儿24h后即给予5%葡萄糖水2mL微量喂养,将喂养后能耐受的定为A组,为54例;喂养后不能耐受的定为B组,为42例,给予禁食2d后再次喂养,有40例能耐受,2例不能耐受。结果窒息组与对照组脐动脉血气分析指标pH、PO2、PCO2、BE,差异有统计学意义(P<0.05);窒息组内A、B两组之间的脐动脉血气分析指标pH、PO2、PCO2、BE,差异有统计学意义(P<0.05)。B组禁食2d后微量喂养,喂养不耐受发生率为4.8%。结论脐动脉血气分析能较准确地反映胎儿出生时缺血、缺氧的程度,在严重的酸中毒时,患儿常有喂养不耐受,应延迟开始喂养的时间。
Objective To evalulate the relationship between pH,PO2,PCO2,BE of arteria umbillicalis blood and feeding intolerance in Apneic neonate of perinatal period.Methods One hundred and eighty-nine neonates borned in our hospital were selected,According to the Apgar score standard divided into two groups:control group in which 93 cases were greater than eight points for the health of the newborn birth;Asphyxia group of 96 cases were lower than eight points of the neonatal;After its birth respectively called umbilical artery blood gas analyzed.Asphyxia group of 24 hours after children that give 5% glucose water 2 mL trace feeding,feeding after the tolerated as group A,for 54 cases.After the feeding intol erance for group B,for 42 cases,to fast 2 days again after feeding,there were 40 patients tolerated,2 cases could not tol erate.Results There was statistical different in pH,PO2,PCO2,BE of arteria umbillicalis blood gas analyzed between control group and apnoea group(P 0.05).There was statistical different in pH,PO2,PCO2,BE of arteria umbillicalis blood gas analyzed between group A and group B(P 0.05).Group B neonates were microcrystalline feed again after abrosia 2 days and feeding intolerance incidence rate was 4.8%.Conclusion Arteria umbillicalis blood gas analysis can exactly reflected asphyctic degree of foetus.It should delay feeding for those apneic neonates whose blood gas analysis is severe acid because their feeding is intolerant.
