摘要
目的研究将脐动脉血气分析用于新生儿窒息预测的价值。方法本实验共选取150例新生儿为研究对象,根据新生儿窘迫情况分成两组,Ⅰ组(n=45胎儿宫内窘迫)和Ⅱ组(n=105胎儿无宫内窘迫);根据Apgar评分的三个梯度≥8分、3~7分和≤3分即分成A组(n=110正常)、B组(n=33轻度窒息)和C组(n=7重度窒息)。所有患儿均接受脐动脉血抽取操作,通常抽取的剂量以0.1mL最适宜用于测定患儿的血气分析,除此之外所有新生儿还接受1min Apagar评分,对三组患儿进行不同指标比较。结果Ⅰ组pH值指标高于Ⅱ组(P<0.05);PCO_2随着1min Apgar评分减少不断升高,两者呈负相关系;PO2、pH值、HCO_3^-及BE指标数据均随着1min Apgar评分的减少而减少,呈正相关系(P<0.05);相比于p H值≥7.20新生儿缺氧缺血脑病发生率,p H值<7.20发病率更高(P<0.05);相比于评分≤7新生儿缺氧缺血脑病发生率,Apgar评分≥8分发病率更高(P<0.05)。结论将脐动脉血气分析应用于新生儿窒息的临床预测当中具有重要作用,通过不同脐动脉血气指标数据变化情况及Apgar评分数据能够用于病症诊断参考,尽早确定患儿缺氧缺血情况,对于制定科学有效治疗方案、提高新生儿生存质量均具有重要作用。
Objective To study the predictive value of umbilical artery blood gas analysis in neonatal asphyxia. Methods 150 neonates were selected as the study subjects and were divided into two group according to the situation of neonatal distress, groupⅠ(n=45, intrauterine fetal distress) and groupⅡ(n=105, no intrauterine distress); According to the Apgar score were divided into three gradient,≥8 points, 3-7 points and≤3 points, group A(n=110, normal), group B (n=33, mild asphyxia) and group C(n=7, severe asphyxia)0.1mL of umbilical artery blood was collected from all the children and used to analyze the blood gas, and all neonates also received the 1min Apagar score. Different indexes in three groups of children were compared. Results The pH value of group Ⅰ was higher than that of group Ⅱ(P 〈0.05). PCO2 increased as the 1 min Apgar score decreases, and PO2, pH, HCO3- and BE decreased with the decrease of 1 min and Apgar score(P 〈0.05); Compared with pH≥7.20, neonatal hypoxic ischemic encephalopathy in the cases with pH〈7.20 was higher(P 〈0.05); Compared with ≤7 points, neonatal hypoxic ischemic encephalopathy in the cases with≥8 points was higher(P 〈0.05). Conclusion Umbilical artery blood gas analysis in the clinical prediction of neonatal asphyxia, by analyze blood gas index of different umbilical artery and Apgar score to early determined hypoxia and ischemia in children, plays an important role in making scientific and effective treatment plan and improving the quality of life of the newborn.
