摘要
目的探讨大鼠染矽尘早期肺组织病理改变。方法 Wistar大鼠随机分为对照组和15、30、60mg/ml染矽尘实验组,每组42只。一次性气管内注入染尘建立大鼠矽肺模型,每组分6个时间点(1、3、7、14、21、28d)分别取7只大鼠处死,取肺脏称重,福尔马林固定,石蜡包埋,HE染色,普通光镜下观察肺组织病理变化。结果各剂量染尘大鼠各时间点肺组织脏器系数均高于对照组(P<0.05),且随时间延长和染尘剂量增加其增高幅度增大(P<0.05);肺组织肉眼观察可见,随着染尘时间的延长,肺表面逐渐出现颜色变深、体积增大、不规则斑块增多、弹性减弱等症状;HE染色显示,大鼠染尘后28d内,肺组织以炎症反应为主伴发肺间质的弥漫性纤维化。实验大鼠炎症反应程度随染尘剂量的增加加重,在染尘后7~14d肺组织的炎症反应比较明显。肺间质纤维化程度随实验时间的延长及染尘剂量的增加有逐渐加重的趋势。结论矽肺早期炎症反应明显,主要表现为细胞浸润聚集,肺泡间隔增厚,纤维组织增生,肺泡正常组织发生改变。
Objective To study the early pathological changes in silica exposed rat lung. Methods 168 Wistar rats were randomly divided into four groups, i. e. the control group and 3 exposed groups(15,30,and 60 mg/ml respectively)with 42 rats in each group. The silicosis models were established by tracheal instillation. 7 rats from each group were sacrificed at 6 definite dates (the 1 st, 3 rd, 7 th, 14 th, 21 st, and 28 th day). The lungs were weighed, fixed in formalin, embedded in paraffin, and stained by HE. The pathological changes in lungs were observed under light microscope. Results The organ coefficients of lungs of rat exposed to various doses sacrificed at any dates were higher than that of the control group(P^0.05). The dose--response relationship was significant(P〈0.05). As the exposure time elongated, the surface of rat lungs gradually darkened, the mass enlarged, irregular patches increased, flexibility weakened etc. The HE staining showed that after exposed for 28 days inflammatory reaction associated with diffuse interstitial pulmonary fibrosis were observed mainly. Inflammatory response enhanced as the doses increased, and hecame more distinctive after exposed for 7~ 14 days. Interstitial pulmonary fibrosis gradually became more serious as the experimental time prolonged and the doses increased. Conclusions At the early stage of silicosis, inflammatory reaction is obvious. The main manifestations are coagulation of cell infiltration, thickened alveolar septa, and fibrous tissue hyperplasia, finally the normal lung tissue changes.
