摘要
目的探讨免疫共刺激分子4-1BB/4-1BBL在过敏性紫癜(HSP)患儿外周血单个核细胞(PBMC)中的表达及其意义。方法 34例HSP患儿分为紫癜性肾炎(HSPN)组13例,过敏性紫癜无肾受累(NO-HSPN)组21例;20例健康儿童作为健康对照组。应用反转录-PCR(RT-PCR),并采用植物血凝素(PHA)和地塞米松(Dex)进行干预,检测各组儿童PBMC中4-1BB mRNA和4-1BBLmRNA的表达。结果 HSPN组及NO-HSPN组患儿4-1BB mRNA(0.921 3±0.229 9、0.855 8±0.235 7)、4-1BBL mRNA(0.440 2±0.118 4、0.407 4±0.076 2)的表达均明显高于健康对照组(0.539 3±0.145 7、0.296 9±0.071 2)(Pa<0.01)。加入PHA刺激后,3组4-1BB mRNA、4-1BBLmRNA表达均明显增高[(1.798 9±0.305 5)vs(0.921 3±0.229 9),(1.660 4±0.306 9)vs(0.855 8±0.235 7),(1.391 4±0.118 7)vs(0.539 3±0.145 7);(0.976 3±0.326 1)vs(0.440 2±0.118 4),(0.906 5±0.254 9)vs(0.407 4±0.076 2),(0.708 5±0.155 8)vs(0.296 9±0.071 2)(Pa<0.01)],且NO-HSPN及HSPN组增高较健康对照组升高更明显。加入Dex后,HSPN组及NO-HSPN组4-1BB mRNA、4-1BBL mRNA表达均明显减少[(0.534 8±0.127 8)vs(0.921 3±0.229 9),(0.502 0±0.105 3)vs(0.855 8±0.235 7);(0.298 9±0.078 0)vs(0.440 2±0.118 4),(0.274 4±0.059 2)vs(0.407 4±0.076 2)(Pa<0.01)],健康对照组的表达减少不明显。结论共刺激分子4-1BB、4-1BBL可能参与HSP的起病,提示二者可以作为监测HSP患儿病情的免疫学指标。
Objective To explore the expressions of costimulatory molecules 4-1BB and 4-1BBL of peripheral blood mononuclear cells in children with Henoch-Schonlein purpura(HSP) and their significance.Methods Thirty-four HSP children were enrolled and were divided into 2 groups,HSPN group(with nephritis,n=13)and NO-HSPN group(without nephritis,n=21),respectively.Healthy control group included 20 healthy children.Reverse transcription-polymerase chain reaction(RT-PCR) technique was applied together with phytohematoagglutinin(PHA)and dexamethasone(Dex) as intervention,to quantify 4-1BB mRNA and 4-1BBL mRNA expressions of peripheral blood mononuclear cells in 34 HSP children.Results Compared with healthy control group(0.539 3±0.145 7),(0.296 9±0.071 2),the expression of 4-1BB mRNA(0.921 3±0.229 9,0.855 8±0.235 7),4-1BBL mRNA(0.440 2±0.118 4,0.407 4±0.076 2)increased obviously in HSPN group and NO-HSPN group(Pa〈0.01).The expressions of 4-1BB mRNA,4-1BBL mRNA in 3 groups all increased obviously especially in HSP after adding in PHA[(1.798 9±0.305 5) vs(0.921 3±0.229 9),(1.660 4±0.306 9) vs(0.855 8±0.235 7),(1.391 4±0.118 7) vs(0.539 3±0.145 7);(0.976 3±0.326 1) vs(0.440 2±0.118 4),(0.906 5±0.254 9) vs(0.407 4±0.076 2),(0.708 5±0.155 8) vs(0.296 9±0.071 2),Pa〈0.01].The expressions of 4-1BB mRNA,4-1BBL mRNA of PBMC in the healthy control group decreased little but decreased obviously in NO-HSPN group and HSPN group after adding in Dex[(0.534 8±0.127 8) vs(0.921 3±0.229 9),(0.502 0±0.105 3) vs(0.855 8±0.235 7),(0.298 9±0.078 0) vs(0.440 2±0.118 4),(0.274 4±0.059 2) vs(0.407 4±0.076 2)(Pa〈0.01)].Conclusions Constimulatory molecules 4-1BB and 4-1BBL may play a critical role in the pathogenesis and development of HSP.4-1BB and 4-1BBL may be utilized as an immunological marker to evaluate the condition of children with HSP.
