摘要
目的探讨矽肺、煤工尘肺及陶工尘肺各期肺功能损伤情况以及影像学改变特点。方法对矽肺、煤工尘肺及陶工尘肺病例尘肺分期及其肺功能损伤类型、肺功能异常率进行统计学分析。结果 0+期与其他各期肺功能总异常率分别为51.47%及70.76%,差异有统计学意义;Ⅲ期与0+期、Ⅱ期间差异也有统计学意义。3种尘肺肺功能损伤类型均以限制型为主,其次为混合型,阻塞型最少。矽肺Ⅲ期中限制型肺功能损伤所占比例减少,转为混合型为主,与Ⅰ期相比,差别有统计学意义;煤工尘肺也有类似趋势,并见肺功能异常率随接尘工龄增加有增高趋势。3种尘肺影像学均以圆形小阴影为主,圆形小阴影组与不规则小阴影组相比,后者肺功能异常率显著高于前者,有大阴影改变者较仅有小阴影时肺功能异常率高。结论约70%的尘肺病例具有肺功能损伤,并随期别增高肺功能异常率增加,肺功能损伤类型以限制型为主;影像学上3种尘肺均以圆形小阴影为主,不规则小阴影对肺功能的影响更大。
Objective To explore the characteristics of impaired lung function and features of pulmonary image among silicosis,coal worker＇s pneumoconiosis and potter＇s pneumoconiosis.Methods The staging of pneumoconiosis,type of impaired lung function and incidence rate of abnormal lung function in silicosis,coal worker＇s pneumoconiosis and potter＇s pneumoconioses were analyzed.Results The results showed that the incidence rates of abnormal lung function in 0＋ stage of pneumoconiosis and the other three stages were 51.47% and 70.76% respectively,there was significant difference.The most common type of impaired lung function is the restrictive dysfunction in ventilation,the next is the mixed,the obstructive one is the least.However,in the third stage of silicosis and coal worker＇s pneumoconiosis,the most common type of impaired lung function is the mixed.The main image change in three pneumoconioses was rounded small opacities,the incidence rate of abnormal lung function in patients with irregular small opacities was higher than those with rounded small opacities,and also in the patients with large opacities compared with those with small opacities.Conclusion The results suggested that the incidence rate of abnormal lung function in pneumoconioses is around 70%,increases with the increase of stage;the most common type of impaired lung function is restrictive dysfunction of ventilation,and the rounded small opacities is the most common image change in three types of pneumoconioses,the irregular small opacities seem to have more obvious effect on lung function.
