摘要 目的:观察胰岛素样生长因子-I(IGF-I)、IGF-I受体(IGF-IR)在人退行性变腰椎间盘组织中的表达,探讨其生物学意义。方法:免疫组化(S-P)法分别检测IGF-I、IGF-IR、增殖细胞核抗原(PCNA)在正常对照组(A组,n=16)、退行性变椎间盘组(B组,n=53)中的表达;TUNEL法检测两组椎间盘细胞凋亡情况。结果:IGF-I、IGF-IR在B组中表达低于A组,两组之间差异显著(P<0.05);细胞增殖指数(PI)B组低于A组,两组之间差异显著(P<0.05);细胞凋亡指数(AI)B组高于A组,两组之间差异显著(P<0.05);IGF-I、IGF-IR的表达与PI呈正相关(rs=0.59,rs=0.56,均P<0.01)、与AI呈负相关(rs=-0.60,rs=-0.57,均P<0.01);PI、AI值在IGF-I、IGF-IR阴性和阳性组有显著差异(均P<0.05)。结论:IGF-I、IGF-IR的异常表达与人腰椎间盘退行性变密切相关;IGF-I、IGF-IR促进细胞增殖,抑制细胞凋亡,在人腰椎间盘退行性变过程中起重要作用,IGF-I、IGF-IR异常表达可能导致椎间盘退行性变的发生。 Objective: To observe the expression of IGF-I and IGF-IR in human degenerative intervertebral discs and to investigate its biological significance.Methods: 69 patients were divided into two groups: 16 cases of normal intervertebral discs in group A and 53 cases of degenerative intervertebral discs in group B.The expression of IGF-I,IGF-R,PCNA and apoptosis were detected by immunohistochemistry and TUNEL.Results: The expression rates of IGF-I and IGF-IR in group B was lower than those in group A,the difference was significant between them(P0.05).Proliferative index(PI) in group B was lower than that in group A,the difference was significant between them(P0.05).Apoptotic index(AI) in group B was significant higher than that in group A(P0.05).The expression of IGF-I,IGF-IR had positive correlation with PI(rs=0.59,rs=0.56,all P0.01) and negative correlation with AI(rs=0.60,rs=0.57,all P0.01).There was significant difference in PI and AI between IGF-I,IGF-IR positive expression group and negative expression group(P0.05).Conclusion: Abnormal expression of IGF-I,IGF-IR is closely related to degenerative intervertebral discs;IGF-I and IGF-IR promote cell proliferation and inhibit intervertebral discs cell apoptosis,which has an important effect on the degeneration of intervertebral discs.Abnormal expression of IGF-I,IGF-IR can induce degeneration of intervertebral discs.
