β_2-AR基因多态性及单倍体型与高血压病并肾功能受损的关联研究

摘要 目的:探讨β2-AR基因多态性与高血压病(EH)并肾功能受损易感性的关系。方法:收集158例EH并肾功能受损患者(EH-RI组),及与之按性别、年龄、居住地相匹配的肾功能正常的EH无肾功能受损组(EH-non-RI组)及血压肾功能正常对照组各158例。应用PCR-RFLP检测β2-AR(-1023G/A,rs2053044;+252G/A,rs1042717)多态性。用SHEsis网络软件估计单倍体型频率。结果:χ2分割显示:EH-RI组β2-AR/-1023的AA频率(13.29%)高于EH-non-RI组(6.33%)、正常组(4.43%)(χ2=8.940,P<0.01)。Logistic回归分析,以EH-non-RI组为参照系,调整危险混杂因素后,β2-AR/-1023-AA型患者EH-RI的OR值为2.561(P<0.05)。在构建的单倍体型中,EH-RI组的G-G单倍体型(β2-AR/-1023—β2-AR/+252)频率(40.19%)低于EH组(47.78%)、正常组(48.10%)(χ2=5.109,P<0.05)。结论:携带有β2-AR/-1023-AA基因型是患EH-RI易感因素之一,G-G单倍体型(β2-AR/-1023—β2-AR/+252)对EH-RI有保护作用。 Objective:To investigate the association between hypertension complicated by renal impairment(RI) and the two variants of the β2-adrenoreceptor gene in Chinese.Methods:EH-RI group of 158 subjects were collected by evaluating serum creatinine level,and matched by subjects of EH-non-RI group with normal renal function and control subjects for individual age,gender and geography.All the subjects were genotyped for the polymorphisms of β2-AR/-1023,β2-AR/+252,and the genetic variants frequencies were compared in all subjects.The SHEsis program was employed to estimate the haplotype frequencies.The SPSS program was used to perform the logistic regression analysis.Results:Partitions of χ2 method showed that the higher prevalence of β2 -AR/-1023-AA genotypes was noted in EH-RI group(χ2=8.940,P0.05).And logistic-regression indicated that β2-AR/-1023-AA genotypes was a risk factor for EH-RI with the reference category of EH-non-RI.The OR of the β2-AR/-1023-AA genotypes vs GA/GG for EH-RI was 2.561 (95%CI:1.102~5.592,P0.05) after adjusting for risk factors of EH-RI.The G-G haplotype (β2-AR/-1023—β2 -AR/+252) was significantly less frequent in EH-RI individuals than that in EH-non-RI group and in control (Partitions of χ2 method,χ2=5.109,P0.05).Conclusion:The β2-AR/-1023-AA genotypes is significantly associated with EH-RI,and G-G haplotype(β2-AR/-1023—β2-AR/+252) may contribute to be a preventive factor to EH-RI.
